Here's a look at Dr. Eric Berg's views on cholesterol and heart disease.

Total cholesterol=not the best marker, but it associates with cardiovascular disease, with heart disease.
2018 American Heart Association Guidelines:
“optimal total cholesterol=about 150. populations with cholesterol in this range manifest low rates of atherosclerotic cardiovascular disease"
scientific advisory on dietary cholesterol: ‘no significant association with cardiovascular disease risk’. this is the science advisory on dietary cholesterol
dietary cholesterol= cholesterol in your food. Serum cholesterol=level in the blood, in our blood work.
He’s talking about serum cholesterol, total cholesterol= total cholesterol in your blood. but the link is on dietary cholesterol
total cholesterol associates with cardiovascular disease risk but there are better markers. LDL-cholesterol is better. nonHDL-cholesterol is even better. apoB is the best. Dietary cholesterol associates with cardiovascular disease in some studies, not others
Lipids travel in lipoproteins. cardiovascular disease (CVD) is about lipoproteins, but most public discourse focuses on lipids, the cholesterol, good/bad cholesterol, but its high lipoproteins that raise risk
lipoproteins=vehicle, cholesterol=passenger. apob=best metric. apoB measures vehicles. lipids, total cholesterol, LDL cholesterol etc measure passengers
sdLDL = harmful, large LDL = fine? high cholesterol ok as long as particles are large??
if 2 populations have same cholesterol, those with smaller particles tend to have higher risk. but for the same cholesterol People with smaller lipoproteins have more of them
Particle size could be the cause of risk or an innocent bystander
at a given level of LDL cholesterol, individuals with small LDL particles have higher risk. but at same LDL-cholesterol, the ones with smaller particles also have more particles
it is the number of LDL particles, not LDL size or the amount of cholesterol, that is the principal determinant of heart disease risk
association of LDL size with risk loses significance when adjusted for particle number, Apo B
apob measures particle number. particle size correlates with risk but after you remove apob, or particle number from the equation, particle size no longer predicts risk
1) statins, lower risk yet reduce large LDL
2) patients with familial hypercholesterolemia have mainly large LDLs, and are at high risk, indicating large LDLs are not benign
All ApoB lipoproteins under 70 nm in diameter, including VLDL remnants and LDLs, flux across endothelial barrier
all ApoB-containing lipoproteins, including VLDLs and their remnants as well as LDLs, have approximately the same effect on risk of cardiovascular disease per particle
the decrease in risk per VLDL decrease was the same as the decrease in risk per LDL decrease. Thus VLDL and LDL pose equal cardiovascular risk
LDL subclass measurement does not add independent info to LDL concentration (number of particles, apoB) along with standard risk factors (diabetes, smoking, BP, family history)
don’t know your apoB? LDL-c and HDL-c are rough estimates but they can be off. when in doubt get ur apoB
‘sdLDL is a marker of inflammation’. sdLDLs are markers diabetes, IR, obesity. they associate well with heart disease because they’re a marker for many issues
Replacing refined carbs (table sugar, candy, white bread, cookies, pizza etc) with unrefined carbohydrate from whole grains or unsaturated fats associated with lower CV risk

Would love to see more of these please. I'd been tempted by much of Eric Berg's advice (his clear concise layout and big claims to improve health with nutrients seem appealing at a glance), but want to ensure I follow accurate advice. There's so much contradictory information on nutrition online, so this open discourse on YouTube is very useful

Great explanation sir! I had my ApoB tested after watching your videos. I have familial cholesterol and I am on statins. My ApoB results were very good. It made me feel better and I continue to eat a plant based diet and exercise and keep my weight normal. Thanks for your great videos!

Thanks for making this video. I was going to ask my doctor for a “fractionated” LDL test to see my “oxidized” (small) LDL levels, but as I’ve been watching more and more videos from people doing vegan/vegetarian/Mediterranean diets, the more I was getting a feeling that I would embarrass myself asking my doctor for this, and this vid put that final nail in the coffin.

I follow Dr Berg's advise and it has worked well for me and my Dr, after reviewing my bloodwork, has encouraged me to continue on my Keto/I.F. path. You come across as a good guy, so I have subscribed to you too!

Great video👌 As a "leyman" it is hard to value the advice you are given as a "patient" or "customer". Apparently there are misconceptions among doctors as well. I always try to find multiple sources for information. When dealing with healthcare workers and doctors i'm not embarassed to go for a second or third opinion when needed. I get that there is a "high pressure" occupation and diagnostics can be complicated. It's important to "realise" that opinions differ with diffrent doctors. In my country out of 100 "physical" doctors visits approx 10 are mistreated or wrongfully diagnosed statistically. That is not unsignificant

The drum beat after 11:25 was a nice touch. :) I've found a few issues with some of the things Dr. Berg recommends/teaches/endorses, however for the most part he's been spot-on and overall he's helped me quite a lot. I've worked with 16 doctors over the last 3 years and I no longer care if a doctor is a traditional MD because frankly I've been hurt more by the MDs than the other types (naturopaths, etc), mostly via prescription drug negative effects.

One question: Is the amount of ApoB/lipoproteins in the blood only important for arterial vessels? Wouldn't having lower lipoproteins in the blood also benefit blood flow in the brain and other regions of the body? For instance, should we expect a boost in long distance running ability if someone lowers their lipoproteins as the reduction in lipoproteins would help their blood flow?

I love your work. After having been a junk food vegan for six years, I’ve finally made the switch to whole-food vegan, and your videos give me the extra motivation to abstain from the junk that tempts me throughout the week. (I get to have one cheat meal every week, which is usually falafel and fries or an Impossible or Beyond burger, so I’m not 100% there.)

Before I went vegan in 2011 I had my blood work done. My total cholesterol was 168. 5 years later 125. Since then it stays below 150.

Hey doc. This is great. Just found this channel, and it is a breath of fresh air.
Question: have you done a clip on statins, pros and any cons? You regularly invoke them as effective tools to reduce serum cholesterol, but are there any contraindications for their use? There are other actual docs or Ph.D. types on YouTube who seem to have an almost visceral antipathy toward them. These same people also seem utterly convinced that LDL is a fake villain talked up by the pharma industry trying to sell statins. They say these things very calmly, but they say them. See, e.g., Dr. David Diamond (Ph.D in biology), and Dr. Sten Eckberg, a "holistic doctor" from Scandinavia. What say you? Personally, I'm less interested in the conspiracy angle than in the health angle, For example, if statins could correct a multitude of dietary sins when it comes to serum cholesterol, why aren't they an easy "get out of jail free" card, allowing indulgence in saturated fats, for example, with little or no consequence?

Just found your channel and appreciate your very straightforward and funny delivery. I did Keto for 5 months and lost the weight I gained during the first two Covid lockdowns. I have always had high cholesterol, as do my parents, but thought it was ok because my « good » cholesterol is high and my ratio is good. You just debunked that. Recently had blood work done and had the following results - Triglycerides 83, HDL 110 and LDL 260. I exercise daily, eat lots of fruits and veggies; however, I do eat fatty fish (sardines), minimal red meat, duck, chicken, and full-fat dairy. I am not overweight, have normal BP and no other health concerns.
I have been researching medical articles and it seems that if you have a good triglyceride level and high HDL, you have a lower risk of heart disease, and that this is a better indicator than LDL. I am sure that this is nuanced, but is this true? After watching some of your videos, I am going to cut down on saturated fat and test again, as well as ask for the ApoB test. I am very curious to find out if my LDL will go down significantly by eliminating most saturated fats, i.e., cheese. I live in France, so cheese is everywhere. 🙂

You make interesting points. I have been on an LCHF diet for about 10 years, and I honestly feel great. My Cholesterol numbers are on the high side and, I admit, sometimes I wonder what the long-term effects will be. Are there any studies on these long-term effects? Cumprimentos!

You put out a lot of very interesting videos.
Could you do one on insulin resistance and glucose levels?
This seems to be one of the largest wedges of contention between the Keto and Vegetarian diets in that the grains and starches (as well as processed foods) consumed by vegetarians spikes insulin and glucose levels causing many types of chronic diseases (especially Type 2 diabetes) and that the Keto diet keeps these, the insulin and glucose spikes low which reduces susceptibility to many of these chronic problems.

Great video but I should mention that I have personally followed some of what he says and found it more workable than advice I had received from doctors. I imagine there are things we don't all understand. Medical errors cause an estimated 250,000 deaths in the United States annually and that is a statistic for 2020. The medical field is not perfect but that said I think you and Dr. Berg are doing a great job bringing what knowledge you do have to the public and that is truly a great thing.

Great video Dr. Carvalho. I think a video critiquing Dr. Sten Ekberg's - " Your Doctor Is Wrong About Cholesterol would be a wonderful compliment to this one. Both Doctors appear to be advocates for ketogenic diets and want to be selective in the supporting studies they cite. Once again great video!

About one minute into this video, I paused to review Dr. Berg's, visited the references, and arrived at the same conclusion. It's easy to see this as an unforgivable disservice, but I view it simply as a mistake. My hunch is that he's addressing the serum cholesterol spike often observed in patients on ketogenic diets -- some of whom he coaches professionally -- and he's attempting to quell a panic among his viewership. (Just a hunch.)
I do not blindly trust Dr. Berg, partly because I know him to practice Scientology, which imposes a huge cash flow demand on its adherents, leaving him with every motive to deceive people. If, however, you view enough of his content, it becomes quite evident that Dr. Berg possesses a broad knowledge regarding nutrition, metabolism and endocrinology which he shares freely. He makes every effort to be diligent in his research, and relies on reputable sources of information. His typos and mispronunciations can be hilarious, but who's perfect? (At least he doesn't say "prostrate cancer!") I accept your criticism on this video, to the extent that it is fair and accurate, however I challenge you to spend a little time reviewing more of his content, and see why so many people have come to trust him.
While Dr. Berg is by no means an infallible information source, I can tolerate little flubs, because I also view content from Dr. Sten Ekberg, Thomas DeLauer and other keto advocates, who cover much the same territory, from different perspectives. I find that referring to multiple resources and reading between the lines helps ensure that I wind up harboring fewer misconceptions. (I also spend on the order of 3-4 hours weekly, scouring PubMed for health information.)
I feel that increasingly, MD's are being reduced to pill pushers. I absolutely believe in allopathic medicine, when it comes to handling a medical crisis, but I'm less impressed with its track record, managing chronic conditions. In particular, general practitioners do their patients a great disservice, by throwing pharmaceuticals at symptoms without first confirming/remediating nutritional status.
Being reflexively handed a prescription for my symptom feels like asking a mechanic to research my vehicle's CHECK ENGINE light -- only to have him invest all of twenty seconds into hiding it behind a strip of electrical tape, charging me $60 for the service, and finally telling me that I'll have to buy a new strip every month, for the rest of my life.

This video is VERY helpful. I have been trying to better understand the entire cholesterol issue for nearly a year and it’s still a challenge. I like Dr. Berg and watch many of his videos. He’s generally good, and he means well, but I never take what he says as gospel. I need multiple sources. I certainly had adopted this idea that LDL particle size matters. More people on YT say it than Berg. Very convincing that it doesn’t really.

Just felt the need to comment in general: finding your videos was something that got me started on a health journey. I started pretty OK; moderately junky plant based diet, good body weight (22 BMI), moderate exercise (swimming and biking), good blood work (total cholesterol in the 170 range).
My family has a history of TIAs, and even though I don't really know or understand the underlying cause (the relatives that got it all had atherosclerosis), I thought that living more healthy wouldn't hurt. I am now eating more whole grain and less junk, and my latest blood work showed a total cholesterol of 125ish (3.1mmol/DL) with better ratio, a LOT better iron and ferritin, and consistently lower CRP (inflammation marker)

what annoys me so much about this topic is the fact that a lot of people are told by their doctor that they need to make some serious changes to not stroke out, and then go on the internet and listen to some quack that tells them what they want to hear just to keep going as they used to. it's the same with the anti-vaxxers basically. i'm so tired of it all.

I know a lot of people who have lowered their Apo B with Keto and Carnivore diets, yet at the same time if googling for how to lower the amount, the diatery guide is always "lower the amount of saturated fats and eat more fiber". So, my question is are there multivariate analyses on studies that show, that
a) saturated fat IS the cause to increased amount of Apo B or
b) a keto or carnivore diet will increase the amount of Apo B?

Thank you for your informative video. I do whatever makes sense to me. I am not comfortable with a high ldl and cholesterol over 200. I keep them low. In order to stay healthy, I need to get Information from variety of sources. Even we’ll intention people get caught on a set way of doing things. Not one diet is good for everyone.

I went on a keto diet and my LDLs skyrocketed. My triglycerides are low, BP normal, weight normal. My family doctor wants me to go on statins. Im really grateful for this explanation about particle size as I was so convinced that I had big fluffy ones and so was safe. So thank you for clearing that up. Clearly keto is not for me.

Total cholesterol is really LDL, HDL, and triglycerides combined. Whenever a person gets a cholesterol test, they may also want to get an advanced lipid profile. This is an advanced test that measures the two different types of LDL.
The two types of LDL are large buoyant and small dense (sdLDL-C). The type of cholesterol that can be dangerous is the small dense (sdLDL-C). The small dense type may be a better marker for predicting future potential heart problems and a better indicator of inflammation inside of the arteries. The small dense type of LDL comes from a high-carb diet.
By doing the healthy version of keto, you can get into ketosis and start burning fat. There will be certain times where your cholesterol goes up and down. But, as long as you keep your carbs low, you’ll get more of the large buoyant LDL and not the small dense type.

So interesting. I just had my blood test done and everything is fine but my dr wants to put me on statins because my LDL ( not the small type ) is high by his standard he thinks it’s genetic. I should have him watch this YouTube video because I refuse to take any drugs. I don’t take anything right now not even a Tylenol and I’m going to keep it that way. I’m almost 70 years old and proud to say I’ve never felt better in my life.

2017 I lost 30 pounds on Low carb and Intermittent fasting.....I went in for a check up and the Doctor said I looked great, but I mentioned because of the low carbs I feel tired. The Doctor did a blood test and I came back 300 on the cholesterol test. The Doctor called at night and had me immediately put on a Statin drug. Over the next 2 and half years I gained all the weight back, starting becoming diabetic, and was having severe memory problems. My NEW Doctor immediately took me off the Statin.....7 months later all the weight came back off and I'm doing cardio 5 days a week and doing great again.

My grandmother and uncle both have naturally high cholesterol AND also happen to be two of the healthiest most active people I know, my grandmother is 92 and still walks, drives, works in her yard etc my uncle is one of the hardest working men I’ve ever met, he has two jobs both of which are EXTREMELY physical...his T must be very VERY high...their doctors want them on statins, I advised against it—don’t mess with a good thing!!

I received a full blood panel and other test to gauge my overall health. My cholesterol level was 204 just a little bit above normal but I still feel that I need to do something about it. My doctor told me my cholesterol level was good, even though my total cholesterol was above the recommended level. I now understand why. This has been very informative. My levels (converted to US measurement) are Total - 228; LDL - 136; HDL - 73 and Triglycerides - 43.

I've been doing keto and IF for about two weeks. Haven't lost weight yet but my knee that hurt following a meniscus tear and repair isnt hurting for the first time in 4 years! Dr berg is almost the only thing I watch now. Everything he says makes sense. My dr freaked when I told him no statins but I'm over them!

Dr. Berg, I would like to thank you so much for this video. I am 38 years old woman who just got clear scans from fighting epithelioid sarcoma stage 4. My ct scan and blood work came back perfectly except for my elevated LDL. I have been doing keto and IF for 3 months now. My cardiologist wanted to put me on the drug to lower LDL, I hate taking drugs and requested advanced Lipid panel testing and just got it done. Now I’m waiting for the result to come back.

You may have saved my life. I just found out that I have severe stenosis in my left vertebral artery. I suspect it all started with a head / neck injury after an MVA long ago. I’ve been having burning in my legs. I’m really concerned. So glad I stopped sugar!

You come across as a bright, informed, engaged professional. In the internet world (as you pointed out), it is impossible to satisfy all of your viewers. You can never prevent emotional reactions from people when you use facts and scientific data that may disagree with their less educated opinion. I find you refreshing and educational! PLEASE never change how you present your information. I am an airline captain, and emotional decisions made in the cockpit are never helpful. Quite the opposite. I have 400+ lives counting on me and my crew to make accurate decisions based purely on factual information. I prefer that passengers like me, but getting them to their destinations safely, far outweighs their opinion of me personally.

I started off watching Dr. Berg last year some time and have enjoyed his content, he seems to provide sensible sounding advice and often backs it up with links to supporting information, studies, whatever. More recently, I've found your channel and it's a real breath of fresh air to see someone pointing out flaws in logic and understanding. Especially after watching people go down covid rabbit holes for the last couple of years. Anyway, after watching this video, I think it's a bit much to expect your audience to verify your claims and the claims of those you're fact checking. I don't think I even knew the difference between serum and dietary cholesterol until I watched your video. How is the average person with little to no science or medical background to begin even understanding what we should look at in these studies? We all have busy lives doing other stuff, we come here to get the gist of the situation, some advice we can "trust", hopefully accurate, and we move on the next video. I work a mentally taxing job and want to switch off for a while at night. We have to be able to trust someone at some level. How can we tell on our own who to trust when we don't know what serum and dietary cholesterol are, or that they're even different things, for example? I certainly don't have time to verify the claims made in every video, I watch 10-15 videos a night easily. I don't have the time or the knowledge to fact check my doctor either. Most of the time I'm watching these vids on a streaming device and can't even see the comments to know if there is any controversy. Honestly it seems hopeless. I do appreciate what you're doing though, very much, I wish there was more of you for many different fields.

Man, thanks a lot for this and the original video. It's so difficult to provide intelligent objective critique as you did. Weather your reaction was too strong - I don't think so. I'm not a medical pro, only an engineer. But my reaction would be the same or much worse if someone mistook physical units and offer his recommendations based on this error. There's no excuse for such grave mistake. Chiropractor or not this should not have happened particularly given the consequences of health decision possibly made by the viewers. Keep up the great job Doc!!!

Also remember that no matter how good you are going to be in particular area you have rights to be wrong from time to time and also you have rights to correct someone on topic which you have understanding of. It's ok to be wrong if you are not misinforming people on purpose or for financial benefits. Same goes for Dr. Berg and for you. You are rational. If listener is also rational then he will understand that noone is 100% right every time.

As someone trained in the scientific method this is a good discussion. One needs to look at biases and motivations which may skew data to a desired result. Dr. Berg often pushes the keto diet and profits from this endeavor. It's important to be skeptical and understand any underlying biases.

I have seen several of Dr. Berg's videos before and always had doubts about his claims that sounded too "out there". As a 35 year old male who has tendancies for high cholesterol, I am extremely grateful that you made that video because it tells me that I really cannot trust someone who confuses diatery cholesterol with blood cholesterol. I will try to do my own research using google scholar paper and try to understand the correlations between these factors and chances of heart disease more (like you showed in your other video - how to do your own research). Wow! The fact that you've turned this into a lesson is crazy! I honestly couldn't even see how it was possible you were "dismissive" or anything negative towards Dr Berg and would've just "dismissed" the people saying that but you actually took on what they said (didn't take it personally) and looked for where it may have come off that way to some people and plan to work on that in the future. Loving your content. You're right, this kind of content is much needed on social media, especially YouTube. I used to watch a lot of Dr Berg but after learning that he is a Scientologist and watching his son's videos I have been majorly turned off.

Knowing no one is perfect in have accepted the experience's and moved on.
Dr Berg comes across as a good honest Dr with a mission to heal.

I like Dr Berg's presentation but I have seen more well placed disclaimer alerts from the many Physical Therapist on here than him. (His is tucked way down at the bottom of his video description) I'm sure he's making lot's of money preaching KETO and knows next to nothing about nutrition, asides his various study references. Mu opinion of him has sunk over the past few months. He's just "one of those YouTubers" that has a big following and cashes in with all that Google Adsense money and whatever teachings/supplements he is chucking

I love your honesty and integrity. I really appreciate that you can admit to making past mistakes and the desire to always be improving. YouTube and social media in general would be improved by leaps and bounds if more content providers followed your lead and if more content consumers would take to heart your guidance on how to analyze claims and to focus on the data and not on just credentials or personal attacks.

Any self proclaimed ‘nutrition guru’ like Berg who preach a very dogmatic approach, absolutely needs to be fact checked. This is the same guy who warned against standing near microwave ovens. 🤦🏻♂️ And I’m not anti-keto or fasting, it’s one of several eating styles that works well for some.

Awesome fact check video. I have been following Dr. Berg for the last 3 months now, it has been awesome and has been working out for me. I also believe in correcting based on facts like you said, this evolves us into the right direction and always theres a room for improvement if its Dr Berg or anybody else.
