Mitochondria control of physiology and disease: beyond ATP

64,853 views Mar 13, 2018 Mitochondria control of physiology and disease: beyond ATP Air date: Wednesday, March 7, 2018, 3:00:00 PM Category: WALS - Wednesday Afternoon Lectures Runtime: 00:57:53 Description: NIH Director’s Wednesday Afternoon Lecture Series Historically, mitochondria have been primarily viewed as biosynthetic and bioenergetic organelles that generate metabolites for the production of macromolecules and adenosine triphosphate (ATP), respectively. The work of the Chandel laboratory has elucidated that mitochondria have a third distinct role whereby they release reactive oxygen species (ROS) and metabolites to regulate transcription factors and epigenetics. For his lecture, Dr. Chandel will present his lab’s ongoing efforts to understand how mitochondria, in addition to producing ATP, regulate cancer and immunity. Genomics approaches to mitochondrial biology and disease | Professor Vamsi Mootha CBMR at the University of Copenhagen 403 subscribers Subscribe 6 Share 246 views May 4, 2023 This keynote was filmed at Metabolism Day on March 14, 2023 at the University of Copenhagen. Metabolism Day is a conference about energy control and metabolism, and is hosted by the Novo Nordisk Foundation Center for Basic Metabolic Research (CBMR). For more information please visit: http://cbmr.ku.dk Vamsi Mootha is an Investigator of the Howard Hughes Medical Institute, Institute Member of the Broad Institute, and a Professor of Systems Biology and of Medicine at Harvard Medical School. His laboratory is dual localized in the Department of Molecular Biology at Massachusetts General Hospital and in the Metabolism Program at the Broad Institute. Dr. Mootha leads a research team dedicated to mitochondrial biology. The long-term goal of his research group is to apply the new tools of genomics and systems biology to investigate fundamental mitochondrial bioenergetics both in health and in disease. Dr. Mootha received his B.S. (with honors, with distinction) in Mathematical and Computational Science at Stanford University. He then received his M.D. (cum laude) from the Harvard-MIT Division of Health Sciences and Technology, where his thesis research focused on mitochondrial energetics. Following an internship and residency in Internal Medicine at Brigham and Women’s Hospital, he worked as a Visiting Scientist at MDS Proteomics in Odense, Denmark, followed by postdoctoral training in genomics at the Whitehead Institute. His research team includes biologists, computer scientists, and clinicians that work closely together. His work has led to the full characterization of the mitochondrial proteome, identification of over 20 disease genes, discovery of all the components of the mitochondrial calcium uniporter, and the unexpected discovery that hypoxia - low ambient oxygen - can buffer against many forms of mitochondrial defects. He and his team have developed computational tools that are widely used in genomics. Dr. Mootha has received a number of honors, including a MacArthur Foundation Fellowship, the Judson Daland Prize of the American Philosophical Society, the Keilin Medal of the Biochemical Society, the King Faisal International Prize in Science, a Padma Shri from the Government of India, and election to the National Academy of Sciences.

I can't thank you, and others in this field, enough for the wealth of information you've provided that help us to make informed decisions when, sadly, our GP's and specialists fail to provide the proper/effective treatment. I'm not knocking them. I’m in UK & I would say the healthcare system here is close to breaking point, not helped by Covid of course, but GP’s have so little consultation time (& mostly now over the phone not in person) I guess it’s just easier to keep dishing out pills instead of being able to get to the root of the problem. Something has to change though as we’re just becoming more over medicated & not healthier plus with an ageing population it’s going to become a bigger problem. Thankfully there are doctors here who are trying to get the message out but it takes time to turn the ship. 2 Reply Roar Ellefsen Roar Ellefsen 1 year ago (edited) Dr. Nadir has a unique way of explaining, he is truly one of a kind, great personality as well. I am known to India, mostly South India where I travelled 1998-1908 a few monts every year, On 8th. of February 2022 , I was having surgery in Copenhagen Rigshospitalet with OHS Mitral Valve w. minimal inscertion, and I am doing good after the surgery and recovering. Yesterday I was invited to last check-up at my local hospital, and they had noticed my Cholesterol was well above treatment border-level. I managed to explain my situation (thanks to Eric Berg's videos): I am trying to loose a little weight or at least stop the body from gaining more weight with intermittend fasting (I never eat animals, I am vegan) and this might be the reason to the high cholesterol. I was so convincing so I got away without any further treatment agrement. My blood preassure went sky high at the shock, just in time for the nurse to meassure, although it was so wrong, because my blood preassure is actually very low normally. I am one of those lucky ones to have what you call "beatiful" veines. No plaque whatsoever there. I keep telling doctors that it is due to my living healthy and vegetarian, that my veines are good. They are so busy with "procedure" that they don't see the person in front of them. During the discussion you mentioned measuring blood sugar and the impact of insulin. There are constant glucose monitors that allow an individual to check the blood sugar but it would seem that being able to measure insulin with a constant monitor would be as useful, if not more useful. If insulin is "pulsed" then a static measurement is not very helpful, as you noted. Dr. Kraft tested thousands of patients and developed his graphs of insulin levels over time. These measurements were taken at intervals over many hours with the equipment available but it would seem that with today's technology it would be possible to get real-time readings to take some of the guess work out of implementing dietary changes. One of the key considerations would be cost for individual use. 6 Reply Roberta Morrison Roberta Morrison 2 years ago Very good information. I hope my MD will watch this for me. Once again I have learned quite a bit and have a lot of take away. I have a comment on the last few minutes of conversation about doctors just subscribing drugs like metformin, statins etc. Perhaps they realize some people just won’t change their diets as well as some people in institutions like nursing homes, schools get mostly potatoes, rice, desserts and overcooked veggies. That’s just one reason I’m trying to keep my mother out of the nursing home. 3 Reply Just Rusty Just Rusty 2 years ago (edited) I changed diet and exercise habits and total cholesterol remained unchanged. HDL and LDL went up and remnant cholesterol went down as did Triglycerides (both cut by about 2/3rds). Doctors freaking out over the LDL and ignoring the rest. (And of course, all my "symptoms" from when I was on statins were attributed to "aging" like Dr. Ali points out. Interesting, I de-aged after I got off statins.) 4 Reply Google User Google User 2 years ago What an underrated channel, thank you for this interview 10 Reply farrokh farr farrokh farr 2 years ago Love this doctor .he is so informative. Second, medication for diseases due to such lifestyle choices need to be abolished or at least severely limited and replaced with nutritional and lifestyle education beginning in grade school. To accomplish all of this, there has to be a paradigm shift which is already happening. Be assured that there will be major opposition by the pill companies, like Merck, and various institutions that will lose billions and billions of dollars when this idea starts to become a reality in the consciousness of the masses. 4 Reply Linda Bladon Linda Bladon 2 years ago Such an excellent and USEFUL conversation 👌. Its nice to have a cardiologist, with such extensive credentials, push back against the lipid hypothesis and the statin drug industry that capitalizes off of this problematic hypothesis. One question though, Dr Ali was recommending a high protein diet, fasting, and exercise when a patient has high tri glycerides so that we empty the fat cell . Why there is a hesitation or not discussed the aspect of having raw fruits and vegetables, which though has carbs but owing to presence of fiber wouldnt it help in satiation, and insulin sensitivity with time along with exercise and fasting? 3 Reply Nadir Shah Nadir Shah 1 year ago Dear Dr Sahib, AOA, Grateful for your advocacy about healthy diet as per statistic of past epidemiological studies. Well, 4 years on and I am still here. 49 Reply 2 replies CheapSushi CheapSushi 1 year ago (edited) His explanation about triglycerides and insulin resistance made a lot click in my head. Thank you. So, if I have this correct, if overweight/obese, it's best to do fasting in some form first, with lower calories overall, in order to get the liver into a better state, less insulin resistant, and then switch over to a more pure low-carb diet after you lose a bit more weight, so excess dietary fat isn't converted to triglycerides by the insulin resistant liver? Once your liver is sensitive again, it can easily handle exogenous fat to convert to ketones? Thanks again. 2 Reply Lucy Burr Lucy Burr 2 years ago I am back (again!) on keto to help fatty liver. Mostly carnivore. I got rid of knee joint pain going carnivore. I stopped eating bread, pasta, white potatoes and etc. Sugar has been tough. But I've reduced it. My Dr. at the VA noted that my LDL is high (yeay!). Well they were internists from a local medical school. They both recommended statins. That freaked me out. So I've been doing research on YouTube. I can almost understand how insulin resistance, electron transport chain, the liver, pancreas, lead to oxidised, and glycated LDL damaging blood vessels in the heart. This is a great conversation. I still get in the weeds because biochemistry is so complex. Reply issatou issatou 11 days ago Great doctor. It pains me that the medical system is still behind. Most doctors I have interacted with still look at LDL, blood pressure, and blood sugar levels. Insulin resistance and hyperinsulinimia get ignored when they seem to be at the heart of almost if not every chronic disease. You see well-intentioned doctors on YouTube making a video on lowering cholesterol and blood pressure when mostly the solution to high blood pressure and high triglycerides seems to lie in improving insulin sensitivity by cutting down junk carbs and bad vegetable oils. It seems that regardless of what we do, we need to cut the sugar, refined carbs, and vegetable oils. If one is diabetic or pre-diabetic, going low carb diet (really low carb of 20-50g) or carnivore seems to be the best path. We need to spread the word and take care of ourselves, our family, our neighbors, and friends. N.B: I use “seem” to indicate that I’m only basing this on the research and the conclusions I come to after listening to people such as Dr. Ali. This is by no means an attempt to provide medical advise or make assertions about medical matters that are beyond my pay grade. I hope such interactions and possibly this comment help anyone struggling with these issues follow up and learn more to improve their condition so that they never need doctors, meds, or hospital stays. Reply Joseph Huether Joseph Huether 1 month ago I think one reason that modern medicine is so oriented towards “treating symptoms” with drugs…wholly aside from lack of knowledge and the influence of drug companies is that it operates in a modern society where we modern people want to “have our cake and eat it to”. They must treat patients “where they live”…which is an environment where they know that only the most committed people will stay on a LCHF diet for a long time. Eventually they’ll go back to drinking beer, Coke, eating pasta 4 days a week and having 12oz of orange juice and a bagel for breakfast. It’s like….yeah…we’d all like to cut our carbon footprint by not using air conditioning…but eventually you’ll turn it back on and therefore we need air conditioning technicians. It’s “where people live” on a day to day basis. He is very good with allowing me to have collaborative care, but he is over fixated on statins. They did great harm to my hubby with cramps all over his body that are just terrible. I suspect he has permanent muscle damage. He was fine on simvastatin, then moved to Atvorstatin , trying to “ get his numbers a little lower”. Before that I mostly dismissed the worries about side effects. But I always felt uncomfortable with lowering cholesterol, since the brain is made of it, but doctors really trust those drugs. How active can you be if your muscles are achy and cramping? Although he checks my fats, he has never checked insulin. I also got fatty liver which I never had over the 20 years diabetic and low carb. My triglycerides were really high. No one could tell me why. I have got very fat on carnivore but got told not to worry after 6-12 months it will start falling of and it hasn't. I'm more prone to gain fat now. High fat carnivore made me very sick and still is making me very unwel as the side effects are still with me. Now I know to lower my fat intake and get my liver and pancreas healthy again. I had to go on insulin because my diabetes went so bad. I got seveare nuropathy in my legs, head and face. Lossing my eye sight. Oral meds wasn't touching it. Now I've been diagnosed as being positive for anti nuclear factor. My body officially hates me and is eating my joints so I can n it exercise. After years of burning carbs your body is looking for carbs and expecting to receive carbs. I think a person on a HFLC diet needs to let there body know that when it's hungry it will be supplied with fat, no more carb. I also think that it shouldn't be to much but when hungry it should be pure fat. I eat once a day and when I get really hungry I have a tablespoon of coconut oil or butter. By eating once a day you are creating a calorie deficit and if you change your body's preferred energy it will burn body fat. 1 Reply 4 replies Muhammad Asim Muhammad Asim 4 months ago (edited) My LDL cholesterol is 153 HDL levels is 55 and tryglicrid is 111 Is this ok I feel so energetic and active My Ha1c is 5.3 Reply A A 1 year ago (edited) So are you saying Dr. that a low carb high fat diet is (good fats like olive, avacado oil and grass feed meat and eggs) is better? But I also need to say that satiety value is important to keeping going with weight loss. So don't be too fast to say casually "cut down fat if you're obese" . There is such a strong (bad) history of dieting by cutting fat!! Also think about female bodies, not just ripped males on bikes. Fasting for men and women seems to have different principles. So while Eat Mostly Fat is the right message, a small part of this discussion (towards the end) left a different impression - - that if you're obese, reduce fat and eat your own body fat. Fine, of course, to be unlocking fat, but you don't want a "dieting mindset" to rear its ugly head again and result in too much protein, or counterproductive hunger. 1 Reply 1 reply liang wong liang wong 1 year ago Hi, does this mean familial hyper cholesterolemia make one longer lifespan and healthy as they have more circulating LDL to benefits them? These findings challenge the widely accepted diet–heart hypothesis. And what is the diet-heart hyothesis?: The traditional diet–heart hypothesis is that particular dietary components (satuated fat) increase blood cholesterol concentrations and that, in turn, elevated blood cholesterol level is causally linked to increased risk of coronary heart disease. In other words, sat fat consumption and cholesterol and LDL levels have nothing to do with heart disease. If there is a fat that has something to do with heart disease, it is triglycerides, which winds up in the blood when liver converts carbs to fat. I don't know if you read these comments, but your videos have helped me a lot, and allowed me to lose a bunch of weight and get off insulin. I did have a significant medical issue this last week, that was related to my super low carb diet. I was going a bit crazy with sodium, adding salt to my eggs, and eating a lot of sausage. Also, a few spoons a day of salted natural peanut butter and cottage cheese, both of which are high sodium foods. It seems that all of this sodium made my BP spike and caused weird headaches.... Now that I've cut back the salt, and unfortunately started taking 40mg Lisinopril qd, things are getting under control... hopefully I can d/c or at least reduce the Lisinopril soon. Just a heads up to people who are going low carb... watch the sodium! 5 Reply Pamee Hanson Pamee Hanson 1 year ago Thanks for making this with simple explanation and examples. My question is how easy is it to inadvertantly drop out of autophagy during a fast? I regularly fast for anywhere from 3 to 4 days up to 11 or 12 days and I feel amazing, have lost loads of weight and have 'cured' my painful wrist, thumb and left knee over the last 6 months. But, I drink one strong coffee in the morning, red-bush (rooibos) tea throughout the day and sip on 2 litres of 'Lo-Salt' water (KCl and NaCl) to stay hydrated. Has the coffee and red-bush (caffeine-free btw) been pushing me out of autophagy? Perhaps someone here might shed some light on this for me... I'd be very grateful. 3 Reply MissLRKindness MissLRKindness 1 year ago (edited) Found your videos, then purchased 2 of your books. Every time I skip breakfast by lunch time my stomach hurts. This happened repeatedly. I figured that I just couldn't skip a meal. It doesn't work for me. Then, I discovered what the problem was. It wasn't the fasting that made my stomach hurt, it was the coffee. Even though I was skipping breakfast, I was still having a cup of coffee with heavy cream and butter. That helped me be full until lunch. But then my stomach would really hurt. I quit the coffee, now, no problem. I have no problem drinking water. Looking forward to the next video. 8 Reply kabes7 kabes7 1 year ago Such an incredible bonus to fasting for weight loss. Would've liked to hear more specifics like how many hours you need to fast to get a good amount of autophagy. The app I use to track my fasting (fastic) says after 16 hours it begins, so I am I right to assume my normal 23:1 regimen is getting me autophagy every day? Lately I've been considering throwing in one or two 48-hour fasts per week since I am not hungry anymore, even at my mealtime. I enjoy eating a lot, but I don't have the physiological need after a 24 hr fast, it's all just for pleasure at this point. So why not try some 48 hour ones? Think I will skip my dinner tonight and see how it goes. 6 Reply 2 replies CHILD OF THE MIGHTY CHILD OF THE MIGHTY 1 year ago I appreciate you doctor. Doing my 36 hr fast starting now. Worst years of my life where i ended up needing b12 and iron injections and doc considered me borderline malnourished. Yes i lost the weight, but i was not functioning. Sleeping 10 h a night plus nap during the day. I got praise for losing weight, but when i tried to tell them i didnt feel better, in-fact i felt worse, i was looked at like I was stupid. Losing weight is “good for you”, right? My son told me i had slurred speech and at times i talked in slow motion. I did not notice this myself really. And now we are talking about my maintenance period AFTER i lost weight. I could after a year maintaining that new weight, STILL not eat more than 1300-1400 calories a day, without GAINING weight. I got fed up and started eating 1600-1800 a day, and felt better. But weight slowly came back even tho i still kept an eye on calories. I was convinced my body was trying to get back to my highest weight and sucked up ever calorie i ate. Watching Dr, Fungs videos put words and scientific explanations to what i lived through. 3 years ago when i started keto to try lose weight again, i felt like a new person. OMAD came naturally due to less hunger, and i loved my “keto brain” as i called it when talking about it with others. I felt creative again for first time in years. I had less pain in the surrounding tissue of my hip joint(which is crocked), so i take less pain medication. I always struggled with inflammation in that tissue being irritated when i moved too much. I dont need codeine daily anymore. I now and then take keto-holiday where i dont count carbs, but it doesnt take long before i want my keto-brain back. Its my default “setting” now..living keto lifestyle with OMAD. Carby days are rare, and only for special occasions. After watching most of his videos i am now experimenting with longer fasting, and i have yet to exceed 36 hours. But so far i love how it makes me feel. So for an over-eater, like i have been since childhood..this is amazing. Hard to believe this is ME living like that. Love it. 5 Reply 1 reply Ochiai Roma Ochiai Roma 1 year ago I lost almost 5 kg for OMAD except Saturday and Sunday for almost two months. I also want to read your next book about why we overeat? Do we seek the pleasure of food because our current urban lives are not as rewarding or engaging as is working on our farm, caring for our animals, tilling our soils? Would we eat less if our jobs were not part of our brainwashing that work is rewarding emotionally when work can never love us back? If there was more sense of community and dedication to love and community-communication about connected things that are emotionally important, would we not seek food as our main reward system? We go to work after eating breakfast. We get coffee with a donut when we get to work. Someone makes a delicious cake we all share in over our lunch break - extended break. We eat lunch to avoid the growling stomach and drop off of stimulants including sugar in the afternoon. We grab a protein bar, a piece of hard candy or even go outside for a nicotine stimulant. On the way home we grab a couple of beers-wine-shot of whiskey. We get home and have an argument with our spouse and go grab a few more drinks. We then eat some cheese and crackers to help with our upset gastrointestinal feelings. We might be lucky and have some meat and potatoes for supper. We might have a few chips with our nightcap. We then fall asleep contented in our fullness. We get up over and over in the night to urinate as our bodies want to dump the sugar carbs of the day. Sounds like food is our only reward. Are we eating too much because our lives are so empty of other satisfactions? 4 Reply 1 reply Raj Raj 1 month ago Does anyone know if Biosteel knocks you out of autophagy??? Or is it going without food on M-W-F? Or eating during the week and only fasting on weekends? Or should everyone be doing a full 24hr fast every 3 months for ultimate benefits? Does one adopt the OMAD or 2MAD forever, or do you stop after you hit your goal weight? I'm just a little confused after reading about so many different ways people fast. I couldn't find any information on this on any channel or blog, they basically all just repeat the information presented here. Say I've been on a 24h fast and let's say autophagy has started and is humming along nicely. Say I am drinking a butter-coffee on Day two of my fast. Will this stop autophagy cold, requiring me to wait another day of fasting for it to resume / start anew? Or will it just set me back a few hours, and after having metabolized that butter-coffee I will just continue to renovate my cells? Most sources speak of needing 18 to 36 hours of fasting to get into autophagy, but none explain how a small snack will set it back. Is it just about nutrient availability in your liver and bloodstream, which can be depleted by exercise, or is it really about signals in your body after having eaten something, anything at all, and those signals simply lasting for that long? Long point short: I enjoy fasting for a few days, but I also enjoy my morning butter-coffee, which is about 250 kcal, a LOT. Will it wreck all the autophagy benefits? Or just set me back a few hours per day. Fasting with that coffee in the morning still feels great, but I'd probably get rid of that too, if it actually rids me of those benefits. If it just diminishes the experience slightly, I don't care all that much. 90% of perfection is good enough for me :) Reply Monique Page Monique Page 1 year ago Dr. Fung, what's your opinion of the "GOLO" insulin resistant supplement? Ppl can do that intermittently as well. I DF daily as long as possible and throw in a longer one each month. Supposedly it increases autophagy up to 3x more than water fasting and you certainly arent hungry during it plus it has been used to heal cancer for centuries. I feel that bc I began full fasting almost 8 yrs ago and learned to dry fast that this is the reason ppl tell me I look half my age, no sagging skin, no wrinkles at 71. Come on, get yr toes wet, pun intended, and pls do one on DFing. Thx Dr Fung!! Reply 6 replies Jenn Spencer Jenn Spencer 1 year ago I wonder if arcana Montana would help autophagy eat waste?
