Heart disease is commonly associated with cholesterol and fat. What’s the science and data say, and is that an outdated and overly simplified model?
Geoffrey Woo sits down with interventional cardiologist, Dr. Nadir Ali to discuss how a cardiologist started applying a Low Carbohydrate High Fat (LCHF) diet in his practice, the nuances of LDL cholesterol, how to properly evaluate insulin, and how healthcare and medicine will evolve in the future.

My husband and I both stopped taking statins. After a few months on them, we both looked and acted 25 years older than we were. Our joints and muscles ached terribly. We started Keto and IF a few weeks ago. We are learning as we go along but we agree, medications should be the last option, not the first.

Dear Colleague,
you are such a nice guy! Your polite way of saying that Big Pharma is blatantly lying and manipulating data is..."the studies are misleading". Thanks for being our beacon of light in this time of darkness.

I am 76 years old and have lost 40 pounds with keto. My stress level were very high from a recent loss. When I lost the weight my energy and brain fog improved but my ldl went up and the doc freaked out and stated "statins" and I said no.

My oldest brother followed his cardiologist's advice to a "T". His wife cooked low fat/fat free, he had developed clogged arteries, had a stent put in one, etc. He had one heart attack! It killed him so quick he didn't even get to close his eyes. He was given a clean bill of health a month earlier. He & his wife were planning a trip to Hungary so they got checked out first. He was taking statins and his lipid profile was excellent according to his Doc.

I did not know that eating was a stressful event to my body. No doctor has ever told me that before and I am almost 70. This is so good to know and keep in mind as a way to help in eating less by way of fasting. Thanks Doctor!!!

In 1991 my cholesterol was 397 at that time my Dr. told me to exercise. Now they put me on statin probably the rest of my life. A friend of mine died when changing his valve his cholesterol was 160 I was confused. Dr Ali is correct and makes a lot of sense.

As a health-conscious, athletic person, now 53, that had to have the Whipple Procedure six years ago (half the pancreas, gall bladder, lower stomach removed and now with a strictured pancreatic duct, taking pills to digest food and insulin injections), interested in not just longevity, but quality of life and physical well being, this conversation was absolutely wonderful!
I can't thank you, and others in this field, enough for the wealth of information you've provided that help us to make informed decisions when, sadly, our GP's and specialists fail to provide the proper/effective treatment. I'm not knocking them. Their hearts are in the right place, but they mask, or reduce, the symptoms, when we need to be addressing the root causes of our metabolic dysfunctions.

What a humble Dr! Someone who’s learning from his patients & realising the usual thinking on cholesterol etc is wrong. I’m in UK & I would say the healthcare system here is close to breaking point, not helped by Covid of course, but GP’s have so little consultation time (& mostly now over the phone not in person) I guess it’s just easier to keep dishing out pills instead of being able to get to the root of the problem. Something has to change though as we’re just becoming more over medicated & not healthier plus with an ageing population it’s going to become a bigger problem. Thankfully there are doctors here who are trying to get the message out but it takes time to turn the ship.

Dr. Nadir has a unique way of explaining, he is truly one of a kind, great personality as well. Can listen to him all day. The study he was talking about on statins in Norway and Scandinavian , have set it's trace. About 650 000 Norwegians was on statins in 2021. Norway have a population of 5,3 million.

Thank You again for your enlightened contribution to our well being and for your explanation about LDL cholesterol to Eric Berg's Video that I just learned about.
I am known to India, mostly South India where I travelled 1998-1908 a few monts every year,
On 8th. of February 2022 , I was having surgery in Copenhagen Rigshospitalet with OHS Mitral Valve w. minimal inscertion, and I am doing good after the surgery and recovering.
Yesterday I was invited to last check-up at my local hospital, and they had noticed my Cholesterol was well above treatment border-level. I managed to explain my situation (thanks to Eric Berg's videos): I am trying to loose a little weight or at least stop the body from gaining more weight with intermittend fasting (I never eat animals, I am vegan) and this might be the reason to the high cholesterol. I was so convincing so I got away without any further treatment agrement. My blood preassure went sky high at the shock, just in time for the nurse to meassure, although it was so wrong, because my blood preassure is actually very low normally.
I am one of those lucky ones to have what you call "beatiful" veines. No plaque whatsoever there. I keep telling doctors that it is due to my living healthy and vegetarian, that my veines are good. They are so busy with "procedure" that they don't see the person in front of them. You are so good, a loving and very intelligent man.God Bless You.

I appreciate the approach that Dr. Nadir has taken, which is asking why his patients have symptoms, getting to the real cause and not just treating the symptoms with medication, the low carb approach, I believe will improve symptoms across the board.

Great discussion. During the discussion you mentioned measuring blood sugar and the impact of insulin. There are constant glucose monitors that allow an individual to check the blood sugar but it would seem that being able to measure insulin with a constant monitor would be as useful, if not more useful. If insulin is "pulsed" then a static measurement is not very helpful, as you noted. Dr. Kraft tested thousands of patients and developed his graphs of insulin levels over time. These measurements were taken at intervals over many hours with the equipment available but it would seem that with today's technology it would be possible to get real-time readings to take some of the guess work out of implementing dietary changes. One of the key considerations would be cost for individual use.

Very good information. I hope my MD will watch this for me. Since our family doctots won't receive this kind of info from their usual sources (big pharma) I feel it's my job to try to educate my doctors and help them think differently when it comes to caring for the people who rely on them for advice. I'm no one's cash cow and I won't go out one-piece at a time!

Great conversation. I wish more people would be open to this. Once again I have learned quite a bit and have a lot of take away. I have a comment on the last few minutes of conversation about doctors just subscribing drugs like metformin, statins etc. Perhaps they realize some people just won’t change their diets as well as some people in institutions like nursing homes, schools get mostly potatoes, rice, desserts and overcooked veggies. That’s just one reason I’m trying to keep my mother out of the nursing home.

I changed diet and exercise habits and total cholesterol remained unchanged. HDL and LDL went up and remnant cholesterol went down as did Triglycerides (both cut by about 2/3rds). Doctors freaking out over the LDL and ignoring the rest. (And of course, all my "symptoms" from when I was on statins were attributed to "aging" like Dr. Ali points out. Interesting, I de-aged after I got off statins.)

No naturopath helped until one autoimmune expert resolved virul load but my Carnivore diet has given me a short term memory, mind that doesnt confuse facts or days, stable energy & happiness that rises from my gut. I lost 35 pounds w/o trying and no friken western medical doctor.

Based on this informative interview, medical schools should begin to focus mainly on prevention and nutrition when it comes to diseases of civilization like heart disease and cancer. Second, medication for diseases due to such lifestyle choices need to be abolished or at least severely limited and replaced with nutritional and lifestyle education beginning in grade school. To accomplish all of this, there has to be a paradigm shift which is already happening. Be assured that there will be major opposition by the pill companies, like Merck, and various institutions that will lose billions and billions of dollars when this idea starts to become a reality in the consciousness of the masses.

Thank you for having Dr. Nadir Ali on your program. I live in the Houston area, and I am proud that we have a cardiologist in our city, such as Dr. Ali, who is willing to look at cardiovascular disease as one that can be addressed through diet and lifestyle instead of just through prescriptions. Its nice to have a cardiologist, with such extensive credentials, push back against the lipid hypothesis and the statin drug industry that capitalizes off of this problematic hypothesis. He has probably done more to promote low carb in our city than any other physician. His 2019 Low Carb Houston Convention was splendid. For those of you who love a more granular discussion on lipids, I encourage you to check out his YouTube channel "Eat Mostly Fats".

Hi, I have just started hearing mr Ali , and your conversations were very helpful to wrap my mind on the fat metabolism, and lipotoxicity. One question though, Dr Ali was recommending a high protein diet, fasting, and exercise when a patient has high tri glycerides so that we empty the fat cell . Why there is a hesitation or not discussed the aspect of having raw fruits and vegetables, which though has carbs but owing to presence of fiber wouldnt it help in satiation, and insulin sensitivity with time along with exercise and fasting?

Dear Dr Sahib, AOA, Grateful for your advocacy about healthy diet as per statistic of past epidemiological studies. Definitely being an interventional cardiologist your advice takes a highest weight. I am a medical doctor and have three stents in place for the last 4 years for over 80% blockage of my CAs and had been taking loprin and Atrovastin. After going through your sessions I have stopped taking statin and only on loprin and following keto diet plain mainly vegetables, butter, nuts and eggs. Just wanted to know if it is safe to still take eggs, butter and animal fat with already existing plaque and moderately raised cholesterol and Triglycerides. Request for your advice , which may also be beneficial to such other patients with NCDs. Dr Nadir Shah, Gilgit, Pakistan

One of the best explanations on the truth of risk, benefit and costs.
Fat is what the body needs.
I had a stroke following a steronclavicluar dislocation, and they tried to put me on Statins. I printed off information and I was told that everything was a lie, and only statins would save me.
Well, 4 years on and I am still here.

His explanation about triglycerides and insulin resistance made a lot click in my head. Thank you. So, if I have this correct, if overweight/obese, it's best to do fasting in some form first, with lower calories overall, in order to get the liver into a better state, less insulin resistant, and then switch over to a more pure low-carb diet after you lose a bit more weight, so excess dietary fat isn't converted to triglycerides by the insulin resistant liver? Once your liver is sensitive again, it can easily handle exogenous fat to convert to ketones? Thanks again.

I was chatting to an old Doc the other day who was saying that it’s a real loss that UK/NHS doctors have quick rushed consultations with people they don’t know vs having a relationship with your patients family-medicine style. Like if you have known a woman since she was a little girl, through multiple pregnancies or ailments, and into advanced age; then you develop a lot more insight and understanding of how to diagnose a problem. Or how you respond to someone not looking and feeling well. Dr Nadir’s approach to working with patients and explaining stuff to them reminds me of the old Doc

I'm LCHF. Mostly carnivore. I got rid of knee joint pain going carnivore. I stopped eating bread, pasta, white potatoes and etc. Sugar has been tough. But I've reduced it. My Dr. at the VA noted that my LDL is high (yeay!). Well they were internists from a local medical school. They both recommended statins. That freaked me out. So I've been doing research on YouTube. I can almost understand how insulin resistance, electron transport chain, the liver, pancreas, lead to oxidised, and glycated LDL damaging blood vessels in the heart. This is a great conversation. I still get in the weeds because biochemistry is so complex.

Great doctor. It pains me that the medical system is still behind. Most doctors I have interacted with still look at LDL, blood pressure, and blood sugar levels. Insulin resistance and hyperinsulinimia get ignored when they seem to be at the heart of almost if not every chronic disease.
You see well-intentioned doctors on YouTube making a video on lowering cholesterol and blood pressure when mostly the solution to high blood pressure and high triglycerides seems to lie in improving insulin sensitivity by cutting down junk carbs and bad vegetable oils.
It seems that regardless of what we do, we need to cut the sugar, refined carbs, and vegetable oils. If one is diabetic or pre-diabetic, going low carb diet (really low carb of 20-50g) or carnivore seems to be the best path. We need to spread the word and take care of ourselves, our family, our neighbors, and friends.
N.B: I use “seem” to indicate that I’m only basing this on the research and the conclusions I come to after listening to people such as Dr. Ali. This is by no means an attempt to provide medical advise or make assertions about medical matters that are beyond my pay grade. I hope such interactions and possibly this comment help anyone struggling with these issues follow up and learn more to improve their condition so that they never need doctors, meds, or hospital stays.

About ten years ago my internist primary care physician prescribed a statin that I dutifully took. (He was the expert, right?) I still get angry about the statin side effects that I encountered for months afterwards (myalgia) and reported to him. He did not tell me that the statin was probably the cause. I had to read the research - and then prescribe to myself the cure for my aches -- stop taking the statin! I am 73 now and still encountering physicians who are concerned about my LDL levels -- and want me to take a statin -- and I just shake my head and say NO.

My skin is cleared up my angina has gone away immensely, I was definitely sugar or should I say insulin resistant I had an insulin problem. All the doctors kept saying I didn't and to this day they still won't give me a glucose test . But I already know since I stopped eating sugar and carbs I eat no less than 40 carbs a day

John Gorman study of 1950. Later stolen then dismissed. Look how much time and quality of life gone because, Drs pushed true medicine away . I’m happier to see more drs looking outside their group for answers. It’s very hard for me as a nurse to stand there and listen to some Drs give bad advice and blame the patient when much of the treatment doesn’t work. High healthy fat is also showing hope in other fields such as ADPKD.

Dr Nadir Ali I appreciate all your YouTube programmes they are very informative but can you explain results off cholesterol like mine 2.79 LDL how serious it really is because my Dr is making it sound like it is very high and I can get a heart attack or stroke if I don’t start my statins immediately. Atorvastatin 10mg was given to me because I am a T2D A1C reading is now 6.5 from 8.6 which I converted without meds absolutely refused my Dr that I will not take it. If there’s anyone reading this is a Dr can you please be totally honest with me and give me the truth as my Dr off 34 years is not answering me correctly. Thanks Suki S 🙏🙏

Please at some point speak to" when menopausal women no longer have estrogen to help, please speak to the metabolic changes and what nutrition is best.
I've had an athletic body and mind set all my life. I've found at 62; incredibly difficult to keep wgt down. What is best food to eat?.

Listening to all the points you make with the disadvantage of taking statins would you think that there is a link between statin taking and high COVID death rates. Statins in the UK are subscribed to almost everyone over 65 as a matter of course. There is even talk about prescribing to all over 50 year olds. The UK has the highest COVID death rate per capita of population especially in the elderly and it is this group that are being prescribed statins. Would be interested to hear your views on this.

I think one reason that modern medicine is so oriented towards “treating symptoms” with drugs…wholly aside from lack of knowledge and the influence of drug companies is that it operates in a modern society where we modern people want to “have our cake and eat it to”. They must treat patients “where they live”…which is an environment where they know that only the most committed people will stay on a LCHF diet for a long time. Eventually they’ll go back to drinking beer, Coke, eating pasta 4 days a week and having 12oz of orange juice and a bagel for breakfast.
It’s like….yeah…we’d all like to cut our carbon footprint by not using air conditioning…but eventually you’ll turn it back on and therefore we need air conditioning technicians.
It’s “where people live” on a day to day basis.

I am going to share with my doctor. He is very good with allowing me to have collaborative care, but he is over fixated on statins. They did great harm to my hubby with cramps all over his body that are just terrible. I suspect he has permanent muscle damage. He was fine on simvastatin, then moved to Atvorstatin , trying to “ get his numbers a little lower”. Before that I mostly dismissed the worries about side effects. But I always felt uncomfortable with lowering cholesterol, since the brain is made of it, but doctors really trust those drugs. How active can you be if your muscles are achy and cramping? Although he checks my fats, he has never checked insulin.

I'm wondering what you think about the approaches being taken by Dr. David Klurfeld, and Dr. Paul Saladino, who are presenting some informations that challenge some aspects of the low carb approach ... I have been practicing the low carb/keto lifestyle for 2 years, and finding a lot of health improvement... Dr. Saladino talks a lot about linoleic acid being a trigger ... Would love to hear you guys discuss this topic.

Amazing talk. I have been keto for along time since Atkins. I got diagnosed a diabetic. I did lose alot of weight but I stalled many years ago so I went carnivore 2 years ago. My fat intake went high like many advise to do. My diabetes slowly got worse and my hab1c doubled. I also got fatty liver which I never had over the 20 years diabetic and low carb. My triglycerides were really high. No one could tell me why. I have got very fat on carnivore but got told not to worry after 6-12 months it will start falling of and it hasn't. I'm more prone to gain fat now. High fat carnivore made me very sick and still is making me very unwel as the side effects are still with me. Now I know to lower my fat intake and get my liver and pancreas healthy again. I had to go on insulin because my diabetes went so bad. I got seveare nuropathy in my legs, head and face. Lossing my eye sight. Oral meds wasn't touching it. Now I've been diagnosed as being positive for anti nuclear factor. My body officially hates me and is eating my joints so I can n it exercise. I wish our community would except differences and that it's not all high fat. There is too much dogma and hate if you don't follow the rules.

tips on how to find a cardiologist who does NOT subscribe to the out-dated statin practice? I moved to a new state and my new gp refused to raise my thyroid med. So this year's #s were even worse (high TSH, body temp 96.6°, 10# gain, higher bp)...and my cholesterol at
350 freaked him out. I'm 74 and active, eat keto, walk, jog, ride a horse, lots of yardwork etc. I am sef-scheduling a CAC scan, but want to find a modern cardiologist to get proper assessment. tx

At 10:16 in, Dr. Ali suggests that your average cardiologist recommends a low fat, low carb diet?! Huh? The average cardiologist recommends the Mediterranean Diet, which is a low fat, HIGH carb diet. Dr. Ali must have misspoken.
At 25:10 in, Dr. Ali suggests that when you lower LDL by taking statins, your brain does not function well. I can attest to that. When I've taken statins, I've experienced depression and poor mental health.

I believe it is not as easy as you think for a obese person to "Switch" to burning his own fat. After years of burning carbs your body is looking for carbs and expecting to receive carbs. I think a person on a HFLC diet needs to let there body know that when it's hungry it will be supplied with fat, no more carb. I also think that it shouldn't be to much but when hungry it should be pure fat. I eat once a day and when I get really hungry I have a tablespoon of coconut oil or butter. By eating once a day you are creating a calorie deficit and if you change your body's preferred energy it will burn body fat.

Love Dr Ali. Brilliant and committed. And spot on. But I also need to say that satiety value is important to keeping going with weight loss. So don't be too fast to say casually "cut down fat if you're obese" . There is such a strong (bad) history of dieting by cutting fat!! Also think about female bodies, not just ripped males on bikes. Fasting for men and women seems to have different principles. So while Eat Mostly Fat is the right message, a small part of this discussion (towards the end) left a different impression - - that if you're obese, reduce fat and eat your own body fat. Fine, of course, to be unlocking fat, but you don't want a "dieting mindset" to rear its ugly head again and result in too much protein, or counterproductive hunger.

Excerpt from 2016 Nature magazine piece:
Newly published data recovered from a large, randomized, controlled trial conducted >4 decades ago show no difference in mortality between individuals with a diet rich in saturated fat and those with a diet rich in linoleic acid (PUFAs/veg oils), despite the cholesterol-lowering effect of the latter. These findings challenge the widely accepted diet–heart hypothesis.
And what is the diet-heart hyothesis?: The traditional diet–heart hypothesis is that particular dietary components (satuated fat) increase blood cholesterol concentrations and that, in turn, elevated blood cholesterol level is causally linked to increased risk of coronary heart disease.
In other words, sat fat consumption and cholesterol and LDL levels have nothing to do with heart disease.
If there is a fat that has something to do with heart disease, it is triglycerides, which winds up in the blood when liver converts carbs to fat.

I can't digest fats as matter of fact the fats I eat they accumulated in my stomach and then I throw it up.
I did keto but kept on having worst issues with the fats. What can I do or use to remedy this.
