Levels Chief Medical Officer, Dr. Casey Means, chatted with our advisor, Dr. David Perlmutter on the discovery of how the elevation of uric acid in the body is a causative mechanism in the development of metabolic diseases, and one of the key links between obesity, diabetes, heart disease, neurologic diseases like dementia, ADHD, fatty liver disease, and …

There was a British doctor in the early 70s that said sugar was bad and he was laughed out of his field - the medical profession owes dr yudkin a huge apology, and needs to be recognized for his brilliant work

My doctor recently told me that the tests I was asking for, during my annual physical, were not necessary because I don't have symptoms. I'm 70 and generally very healthy. I told him that I had been to the endocrinologist a couple of years back and he told me to watch my TSH levels. They came back very hi. I begged the doctor for some of the tests that Dr. Berry and his colleague recommend as part of a health screening and he said no. No Uric acid test recommended. So none done. I just can't wrap my head around how these doctors practice medicine. My experience has totally been that they don't want to find out what's wrong they just want to cram a pill down your throat and damn you if you challenge or question their approach!

OMG! I'm a medical professional and just hearing all this just blew my mind! To say understanding how uric acid affects these multiple pathways in the body is starting to tie up a lot of loose ends regarding the pathogenesis of these diseases is an understatement. It's huge! You guys are like the "Jedi Knights" of the medical world! Thank you for your dedication to learning how these complex pathways work.

This was a superb video (for me). I’ve been obese for years. I was slender all my life & when I began to feel the shift , told my family physician I thought my metabolism had changed. She said the metabolism doesn’t chance. I’ve taken ALL the shame & blame as I blew up. Now I’m 73 yrs old, a former dancer shame makes me unwilling (and unable) to walk or even stand without pain. I swim to get things moving . But I absolutely crave carbs. I drink smoothies (organic fruit). I’ve been told I just let myself go. I’m a shame. I have to “face this “, etc. I can assure yo shame or being yelled at daily has not worked. I DID, however, buy a glucose monitor when I became interested in intermittent fasting. Recently I found the two medicines I WAS ON caused weight gain (1) and the inability to lose (2) weight. So again, what I assumed was my weak character flaws & failures while water fasting were perhaps exacerbated by simply not understanding. Namely, the role of sugars as presented here. Get that monitor and starting over. Wish me luck. Need both books (gave his grain brain book to a neighbor who was diagnosed with Alzheimer’s ). Thank you for your guest today. Thank God for recommending I watch it.

When we took sugar out of our diets 25 years ago because we saw it affected our children's behavior, we were viewed as mean, even abusive parents. Thank God for mean (and informed) parents.

I've been telling people sugar is the problem and not fat for 12 years and people called me stupid. It's nice to see science and doctors finally seeing and talking about it, it's sort of a vindication. Everyone needs to be taught this and to understand that children needed to be protected from sugar as it is highly addictive and unlike heroin its on every grocery store shelf. There is no such thing as an essential sugar......

This is one of the best discussions I've ever watched on YouTube. I have some of the health issues discussed in this video and I've been looking for answers to questions for some time, and this pretty much answered all of them. I paused the video part way through to buy Dr Perlmutter's book. I'm looking forward to reading it, to learning more, and to improving my long term health. Thank you to you both.

This is an example of what SHOULD be taught to us in schools.
None of us left school with any knowledge that was actually important to look after our bodies, our minds, our planet and each other.. if we learnt the important stuff, I'm pretty sure the world wouldn't be in such a mess 😔🕊️

My whole line on my father's side had gout going back to my great grandfather. Gout was accepted as something that was matter of fact. My first attack caught me off guard at age 42. BTW I'm 75 now. Years went by doctors never treated me for high uric acid. I was finally put on Allopurinol 100 mg/day 12 years ago and still had a gout attack. I'm now on 300 mg/day of Allopurinol. I'm a controlled type 2 with cardiovascular disease. Looking back, my doctors never considered gout as an important disease. This discussion is so eye opening and needs super exposure to all the practicing primary care doctors.

Wife and I went on a binge (sweets, chips, etc) and I wound up with Gout. I took organic chemistry and biology in college, so I wasn't lost in the weeds, but this video should be spread far and wide (like our bellies). We're going back on our Atkins diet, but getting a broader picture of what is really going on, is a great help.

Damn, yes!! This presentation effectively ties together vast amounts of research literature into an essential overview of the personal, and public, status and trends of the public 'health' paradigm. Thank you for this 'crazy' peek into biochemical pathways clarifying the metabolic (and mitochondrial) disease crisis, connecting a lot of dots in my mind about the puzzling array of observations of society and environment complexity seen through a lens of evolution throughout millennia.

I found your conversation very enjoyable. I am a Certified Iridologist and have been in practice for 40 years and I'm also a Certified Fitness Specialist and I've spoken to countless clients over the years about the uric acid levels seen in the Iris of the eye. And many if not all had gout, and or suffered many of the dis-eases in the body you spoke of just from high uric acid levels in the body. Gotta give you medical doctors a lot of credit here for telling the truth about our biochemistry, sugar/fructose consumption in this country, poor diet, lack of exercise, and alcohol abuse, plus not exercising self control.. the amygdala VS the frontal cortex. I'll watch this one again for sure. Great information. Good job.👏👏👏

Fascinating podcast, well presented. Thank you for bringing this to public attention.
What astounds me is how something so logical and dare I say obvious has gone under the radar thus far.
I'm based in the UK but expect similar problems exist in the USA. Go back a few generations. People generally ate to the calendar, seasonally if you prefer. Root veg in autumn/winter, blueberries in July, Apples in late summer etc. Nowadays we can buy blueberries for example all year round. I see an environmental link here. Save the unnecessary food miles, eat locally sourced seasonal foods where possible. It is what nature intended.

Thank you for such a comprehensive interview from the good doctor. This information is what I have needed for many years. I developed an addiction to sugar in my thirties and now have been obese for twenty of my 76 years. Looking in the mirror has been a source of shame and frustration that I blamed on my lack of willpower and discipline. The medical community only reinforced that opinion with comments to "not eat so much". Now, after being diagnosed with pre-diabetes (my doctor suggested eating more beans) it seems logical that I need to reduce my sugar intake to almost zero and increase my protein intake as well (more beans). This video confirms that my decision is correct but also points out that reducing my sodium level is just as important.

Great video! It really helped broaden my understanding of gout and uric acid, as well as the other problems and sources of uric acid! I could follow along but it would be helpful if Levels were to create a simplified version of what he's teaching so that the masses who aren't good at science, or who don't have enough knowledge or intelligence to follow along can get the message easily.

She didn't have the medical words for what she observed happening in the body...but her theoretical conclusions were/are spot on to what science has defined and recognized. Wondererufl!

One copy is going to my Mayo physician. Prior to a move to Florida, I had similar discussions with physicians at the Cleveland Clinic on uric acid and its relationship to fat and hypertension.
At the annual physical last month, I mentioned Alexander Haig's name as well as your name. No recognition of both names occurred. I will make sure that does not continue. Thank you.

This is an eye-opening podcast. The conversion of glucose to fructose is a new discovery to me. I read DROP ACID and learned a lot from that book. I have a few questions. 1) Should we fast OR not fast before taking a uric acid test? 2) Is the home rest for Uric Acid as good as the blood test for uric acid taken at the factors office. 2) How long does it take for the salt in a high-salt dinner to get diluted from the body if you normally drink 8 glasses if water? 3) will a high-salt dinner at 8:30pm the night before affect the uric acid test result taken at 10:30am the next day. Thank you!

I quit sugar for new year's along with HFCS, wheat, corn and rice products. I feel great, 45lbs. lighter. I reduced comorbidities and save money on food as I no longer feel the need to eat every few hours. This is important with the food shortage coming down the road. Please use that in your lectures. Harden ourselves by going sugarless, grass and oatless.

I suffered from high uric acid early onset of gout. My doctors were absolutely fucking useless in treating it. Only reason I found out I had it was I looked at my own blood work were it was indicated for 1 year straight, high uric acid.
What I did was removed the unnecessary drug, a diuretic, and problem solved.

Australian Functional Ingredients
Australian Functional Ingredients

I thoroughly enjoyed the presentation. Thank you both.
My research to cure/deal with my own congenital gout has yielded some interesting findings. It might also be the missing link you mentioned. I have been working on the nutritional value of wild foods of the world's longest living culture - the Indigenous Australians.
A few of the findings have lead me to classify sugars as good and bad sugars in a general sense. Good sugars are typically components of antioxidants, anti-inflammatories and anti-carcinogens. They are highly functional. You mentioned quercetin as an important anti-inflammatory. It forms the glycosides quercitrin and rutin together with rhamnose and rutinose, respectively.
Good sugars are easier to define if I explain that what I deem bad sugars are generally the added dietary sweeteners; sucrose and fructose or these sugars bred into super-sweet cultivars of fruits and vegetables. Mangoes and pineapples are so high in bad sugars that their content can be up to 3% higher than those famous cola drinks. A sweet corn variant has been bred with 45% sugars, mostly bad and it is so sweet the common name is candy corn. Tomatoes are are also targets for similar selection.
In contrast, not only are most wild fruits not as sweet but they are high in organic acids (anti-cancer and antioxidant), dietary fibre (unlike modern equivalents) and low in water which makes them nutritionally concentrated. They are particularly high n phytonutrients. See this blog post for a short video on the nine main classes of phytonutrients https://cherikoff.net/phytonutrients-for-us-defense-compounds-for-plants/
The comments made in the presentation on fruits being good sources of fibre and vitamin C are incorrect. Agronomists have long been focused on species selections to improve post harvest cold storage life; durability in distribution chains; resistance to fungal and insect attack; root rot resilience; adaptations to geography; and ever-increasing yields and all these have been at the expense of the nutritional value of the edible portion.
Vitamin C is often quoted as a component and a reason to consume modern produce. Unfortunately, this vitamin falls victim to fertilizers, irrigation and chemical yield boosters as well as food preparation. Peeled and cut fruit mixtures are generally devoid of vitamin C and microbial contaminants consume much of the remnant nutritional value that has not been oxidized through exposure to ambient conditions. Remaining phytonutrients would be fully utilized in countering the negative effects of the excess bad sugars rather than addressing our metabolic needs in cellular free radical quenching and mitochondrial support during ATP production.
Incidentally, an excellent diagram for fructose metabolism in the liver and the uric acid, nitric oxide and other reaction product implications is well presented on page 17 of this useful slide compilation: https://old.diabetes.or.kr/new_workshop/201301/file/pdf/S13_%b1%e8%c1%be%c8%ad.pdf
Not to go on too long here but my solution to my congenital gout, easy weight management through addressing Hidden Hunger (phytonutrient deficiency) and the process to address our rising CAC scores from way too much calcium and being magnesium deficient while also tackling most of the diseases of nutrition developed into 3 base products. I do have one extra anti-inflammatory to take aim at metaflammation and it does this incredibly well.
All are wild food enhanced, phytonutrient rich and heavily science based since I always wanted to market them as functional foods with strong food:health relationships that pass the regulators here in Australia. The results have been astounding.
13 terminal cancer patients living in remission.
Indigenous and non-Indigenous Type 2 diabetics free of any symptoms and biochemistry markers of T@D within 6 weeks.
Hypertension normalized in 2 weeks.
Coronary artery stent insertion made redundant in 4 weeks or less
Kidney stones, gall bladder stones, calcified tendons all gone or greatly reduced (DeQuevain's Syndrome as a long term condition is taking a while to beat but I am winning)
Weight loss without any will power needed and so far, no weight gain for 5 years and counting
Gout-free and enjoying beer, red meat and seafood
Daily intermittent fasting and only 2 meals with total satiety proving that Hidden Hunger for us and our gut bugs is all about phytonutrients
Anecdotal feedback on improvements in MS, ADHD, depression and anxiety symptoms on-going
and a whole lot more.
I am convinced that the nutritional quality of the food supply in Western nations, particularly needs wild foods introduced to counteract the decay of the last 50 years. Calcium is key and probably underscores most of the diseases of nutrition in their early predisposition, development over time and final expression after 40 years of age. Whole food sources of magnesium and critical nutritional considerations (or I offer an exceptional whole food/wild food blend that works.
I would love to collaborate/communicate directly to explore my solutions and yours. Kindest regards.

A couple of years ago I summarized my ailments to include gout, rising blood sugar levels, high cholesterol, weight gain, elevated BMI index, and realized all were related to dietary habits. After all, if one were to stop eating none of these poor readings would be present. So the key to fixing these threats would be to modify food consumption to mainstream recommendations (sugar removal and low carbohydrates) and implementing periods of fasting.
I’m still working on it.

I am female age 62 and have each 10 yrs gone up a size - I am 2 sizes higher than I should be. It’s steady at that size but cannot lose it. I realise that after this video that dehydration is likely my enemy. I eat OMAD one meal a day - I am not hungry before I eat. I don’t add salt and avoid salty food. Not a fruit or sugar eater. I eat healthy foods but I don’t lose weight. My doctor in passing said we will keep an eye on your uric acid as you get older. So why not do something NOW ? I will get Quercetin ? and take 500 mg of Vit C and drink water lots more water ! Let’s see if that helps. Thank you - great video.

I really really wish there had been a discussion about artificial sweeteners. People use them to avoid all of the sugar and empty calories, but from what I've been reading, you don't escape the effects that sugar has on your body given that it tastes sweet and therefore your brain reacts the same way as if has received sugar and sends out insulin in the same way and can make you insulin resistant just like sugar can only faster since people are likely to take in more artificial sweeteners due to their zero calorie nature. I'd like an expert take on this because I do suffer with gout attacks and normally I know exactly what I did to trigger the attack, but my latest one seemed to happen completely unprovoked. Not only that it was clearly the worst gout attack I've had to date and I've had gout for about 10 years now. The possible differences I saw between my last gout attack earlier this year and the one in May were that I had a major uptick in the consumption of diet soda (Coke Zero) and because I workout I had consumed numerous protein bars and shakes which both claimed to have almost no sugar. I have immediately stopped consuming any diet soda and protein supplements cold turkey in light of this theory.

I'm curious to know how vegetable oils fit into this story. Canola oil especially. Aside from sugars detriment to our health, these manufactured oils of all kinds have been proven to be a major contributor to major health issues.

I found this a very enlightening podcast - and great to have an 'evolutionary context' to help unravel some of the bio-chemical mechanisms which lay people like myself have a limited grip on. But this all made great rational and 'well-researched 'sense to me - especially as I have often wondered why my 'uric acid' levels were often elevated in general blood tests - although medicos never commented and thought it 'not an issue'.

I've had an indwelling catheter for the last 10 years and several urologists have ridiculed me for changing the catheters too often. (supposed to last 16 days but I had to change weekly because they started burning my bladder). A few months ago I eliminated excessive sugar from my diet and started drinking black tea (Earl Grey). Problem solved! These urologists were taking urine samples, but not checking uric acid?

I've been battling gout for about a year now, I was eating a lot of bacon and meat in general while running between 35 to 55 miles a week. I was training for a faster 5k time and have dealt with the toe nail issues over the years. I ate a large plate of liver one night and woke up with the most severe big toe pain. I've cut out most things that contain purines and have switched from running to bike riding and lifting weights. Things are getting better, learning about the dangers of uric acid is a big wake up call. Thanks!

Dr. Perlmutter-- I bought your book "Drop Acid" and also a uric acid home test kit. I was unable to reduce my uric acid with diet alone, but after monitoring my uric acid and my blood pressure I was able to demonstrate a direct correlation between my uric acid level and my blood pressure. Now on Allopurinol, my blood pressure has come down from 149/82 to 128/72 or even on occasion, 109/68. (I should include I am type 1 diabetic for 56 years.) The proof is in the pudding as they say. You are 100% correct! Thank you!

Uric acid &Gout , what hasnt been discussed is the relation of Uric Acid and Kidney stones. I have suffered from both and my doctor and urologist are constantly trying to figure it out. My uric acid levels are constantly elevated and it seems hereditary in my case as my father has kidney stones, although no gout. Interesting video. Liked

had a documented heart attack 11/12 august, stint n agioplasty morning of the 12th. triple bypass 14th november, i listened intently to your podcast. two years ago i went to a dr because i had a gout attack. he proscribed some drugs, but also told me what foods to avoid. i did the food route. as a truck driver i drink almost nothing but caffienated coffee when driving. it's a diruretic. that activity is what caused my blood vessel blockage. i've been reading the labels of my food purchases and fructose corn syrup went out long time ago, beer went out after my gout issue, coffee will go down to one cup a day. do i have to cut back on dried fruit?

I happened upon this video by mistake, however, I'm now certain it was 100% meant to be. I've shared it with a little group of ladies where we've been trying to motivate each other to hydrate and walk more...on a simple level...even if we can't do low carb and militant workouts just yet. These 2 notions, though simple sounding, have proven to be rather difficult for me and a few others. It's so odd, isn't it? I appreciate the info here and this further inspires me to not give up on my hydration and walking goals. All the research I've done always seems to come down to more water, move more...and then all the other "fancy stuff" like watching your sugars/ carbs, etc. So, if I could tackle the simple stuff first...I'm already helping my body and mind in leaps and bounds. So, thanks for this kick in the tail. Sparkle on! ✨

I believe in traditional Chinese medicine we are asked which we prefer salty or sweet? I am a 'salt person and not proud. It too is like an addiction to add more flavor.

Outstanding interview. There is the rate of knowledge and it's spread, and that of special interest and the their evil marketing departments to keep new knowledge under cover to retain profits. My Dr. always quickly prescribed a medication - blood pressure and then uric acid - and never took time about diet even when asked. Fast forward and I changed my diet, lost weight, went off most medications... After sharing what's I did regarding diet, he said that it was excellent. But for the vast number of people he sees they just want a pill and never want to address their diet. I have a new Dr, but I'm sure he's right about people.

My brother has had gout 10 times and I've had it 3 times a few years ago until I lost 40 pounds on a whole foods diet. I cut out all sugar, flour and bread. I also got off of amlodipine which is the same bp meds my brother was taking too. So many problems with that particular meds. I have kept the weight off for three years now, take no meds and occasionally eat bread now. I will always have to watch the carbs but I've educated myself on good fats and fat burning and how bad sugar and carbs are. My mother also had gout when she was alive. Bad eating habits all around. You have to educate yourself to save your own life.

He never asked what I ate or when I ate it or even suggesting seeing someone that could help me fight the sugar urges and eat healthy. Even my oncologist (I have CLL) never asks me about my diet or tells me what I should and should not eat with my disease or says anything about me losing weight to be healthier. They just don't care

I have no medical knowledge but following a crash and reading a book by a woman who was in a wheelchair young from arthritis and decided to stop all the meds and use researched old remedies on diet. The key ingredient was Cider vinegar and honey in warm water first thing every morning. Within a month of being off the meds and a diet she worked out she walked for the first time in ages.
So to avoid getting arthritis in damaged joints etc I started taking the cider vinegar honey in warm water every morning. What happened was that my acidic stomach and bowel disappeared and never came back. Before that just a cup of coffee was enough to cause me a problem. An older neighbor who had a few arthritic joints decided to try it and told me that a surprising side effect was that cramps in his legs at night had stopped happening. He only ate fresh organic food and didn't drink or smoke so he had a healthy duet already. So I suggested to another elderly neighbor who suffered badly sometimes from gout to also try it, and wow, it also reduced his gout to almost zero. It came back if he stopped taking it. I don't know how it works or if relevant to this discussion, but I'm just sharing my experience with cider vinegar.

I have been a gout sufferer for a very long time (17 years) and I have always been told by doctors that uric acid is a cause of gout and that allopurinol was the answer. However I have had attacks when I have been taking allopurinol and the foods and drinks which I associate with attacks are wheat based foods (especially baked goods) and alcohol. But when eating lots of red meat and purine high seafood particularly my favourite salmon I get nothing. I tried a highly carnivorous diet for around 6 months before adding in a few veggies I happen to like and recently went full carnivore again and this works superbly well for me. I haven’t had a gout attack in well over a year when back when I was eating flour, sugar and drinking alcohol I got them relentlessly.
I have to say I find the associations mentioned that say uric acid is associated with gout and that the foods listed in the diet section that are high in purine should be avoided to be troublesome. If this was true would not the Masai in Africa and the Inuit in the arctic be absolutely unable to function without almost constant gout attacks from their massively purine-rich foods? Red meat and high fat seafoods are their primary diet and they have zero access to any ‘uric acid’ reducing fruits and vegetables. The last time I looked the Inuit with their almost zero incidence of heart disease were not farming broccoli in northern Canada.
Dr Perlmutter is focused on fructose throughout the book and I see the connection with our high sugar diet in the west, but again I don’t see the direct causal connection between uric acid specifically and gout. If Dr Perlmutter and his colleague have access to a strong double-blind trial where patients were taken off sugar, carbs and alcohol but remained on high purine foods and so as a result had constant gout attacks I would love to see it. I think that would strengthen his case. But this seems to be a very poor association study. Whenever he discusses fructose and gout I see the connection, but uric acid specifically?
It seems rather more to be the case that if you have a poor sugar/carb/alcohol rich diet you will have a high incidence of gout attacks, as well as elevated uric acid, as well as be more likely to die from all cause mortality because of the damage done to the body through a damaged metabolism.
I must say I did find the evolution aspects very interesting and that uric acid was associated with our ancestors ability millions of years ago to store fat was one of the more interesting aspects of the book. However I do think again that in this case I didn’t see too much specific evidence of why uric acid specifically was a causal factor. If our ancestors were eating high purine foods, gaining high incidence of uric acid and then having many, many gout attacks would we also not have many thousands of incidents of gout / arthritis in the fossil/bone record? The response to that would be that our ancestors were not eating massive amounts of fruit, but in that case why then have a list of foods in the book that focus on the reductions of purines and uric acid? Further - if purines and uric acid were also causal for gout - wouldn’t primarily meat eating humans die off because of their constant gout attacks preventing them from hunting for days or weeks at a time?
I’m just not convinced. I see the sugar connection. I don’t see the uric acid connection.

My grandmother walked everywhere she went, ate mostly out of her garden which took up the better part of her back yard and worked in that garden often. I remember her taking the peels and veggie scraps out with a shovel and burying it in the garden. She drank milk, ate butter, made homemade bread every week, baked cookies and cakes and would eat the fat off of the roast along with mashed potatoes and gravy. Breakfast was eggs and bacon when or oatmeal. She had "juice glasses" that would maybe hold a half a cup of juice that she would squeeze from oranges. She had sauce dishes that held homemade apple sauce and other fruit sauces. Meat and milk and dairy were from local sources. She died one month before turning 104.

WOW I think the universe led me to your video today. I just had my first experience with gout and I was so confused as to how it is caused and what I can do to lower my uric acid. Minimal meat.

My first gout attack was triggered by red meat and alcohol. From there I had recurring attacks. Initially “cleansed” my body by eating vegan and only drinking water for 3 months. Ultimately found I can continue to eat red meat and sugar if I avoid high fructose corn syrup (usually in soda) with the meal. I wait several hours before having soda and drink lots of water to flush my system. My last Uric Acid measurement about 2 years ago was high - yet no gout attacks. After watching this video my plan is to drop soda and minimize high fructose corn syrup and see if I can optimize my uric acid level. I don’t drink alcohol at all.

She said all of these test are not like they were, everything is good or better then good “how did you do that” . I don’t take any medicine and it all started with cutting sugar and then work on everything I learn about healthy living. Had gout now I have mobility in both big toes, no gout. In my case, I was not eating foods that were high in sodium, but my legs started to swell at night. My doctor had told me to drink more water, but I stuck to the tea that I enjoy so much. Swollen legs were alarming to me, so I switched my liquid intake to 90% water each day. After about a week of significantly increasing my water intake, the swelling in my legs left and has not returned.

Thank you! Great info. I have had great results with Quercetin and lowering inflammation in general with myself and others. Also with Vit C (reduces oxidative stress) and various forms of Carnitine increasing Nitric Oxide levels. I do believe there should be more research done on supplements in general, rather than hitting everything with 'sledge hammer' pharmaceuticals. Their motivation being financial and of course, supplements do not make the grade. I have been seeing NDs for over 40 years. This is not new news to them. However my MDs look at me like I have 2 heads, extremely bias, indifferent and just plain uneducated in diet, nutrition and supplements. In a perfect world these two modalities should be working together. Too much money to be made.

I have been dealing with gout for the last 30 years and after avoiding sweet foods and high protein foods, my blood pressure is much better and my joints are not as stiff. Alot of deposits around the joints especially in my feet have disappeared and the range of motion greatly improved. After years of taking medications to deal with gout, I am finally free of pain and has lost almost 10 pounds after 3 weeks of this diet and no longer taking any gout medications. My diet was just plain white rice and oatmeal three times a day and I drink black coffee without any cream or sugar. My blood pressure was back to normal and I stopped blood pressure medication too. The only problem is that this diet was extremely difficult to keep and my cravings for steaks and sweets finally broke this diet and now I am back on the medications again.

This explains why on my worst bout of gout a year ago. For a time I was into all kinds of fruit juices thinking it was good for my body and also eating peanuts as snacks. And after my bout with gout, I stopped all fruit juices and took water instead, while still having peanuts. I never have the bouts again since then, but after watching this video, I thrash all my peanuts because of relatively high purine content, opting to almond instead. I feel my action of last year is validated by this study.

THEN i started to fast. i have started with 3 day fast then all the way to 7 days. i did this for 9 months with no problems. after a 3 month break i did a 3 day fast and got the gout in my knees and it seem that my thighs got so tight i could hardly walk. but i did not put it together as gout until i did another 3 day fast and could not get pass the second day with the same deal of knees and thigh pain (very hot in both ). i believe that the more weight that you loose the more uric acid that is released from the adipose tissue. i lost 90 pounds and it seems the closer i got to my weight goal the shorter the fast. so this last month i did a 3 day fast with no problem and the next week i could not get pass 2 days again and i have been achy ever since. would like to see same research on the storage of uric acid in the adipose tissue. you are correct about the meat. learn that in 1980 in the first book on low carb diet by dr. Atkins.

Having Gout was my introduction to uric acid, funny that most of my flare ups would revolve around dropping the fat in a short amount of time.
I am glad your speaker talks about the needed function of Uric acid but not just that but insulin resistance, cholesterol, inflammation other natural functions as survival.

Thank you so much. I’ve listened to this 3 times.
I had my Uric acid level checked and it’s high!
High at a fasted state… it must be horrible at an unfasted state!
Ughhh I’m so up against a wall!
I have hashimotos that is getting worse, diverticulitis, high Uric acid, and I’m obese and have tried to lose weight so many times and failed!
Ughhhh! I want this weight off more than you know and I want to be healthy!
I also have a low thyroid and post- menopausal.
My last problem is that I can’t stand beef, seafood, or pork.
Limits me so much…

Has Dr Perlmutter ever said exactly what he eats? Do they ever share what they eat? He looks healthy. I’m in a new relationship with someone with severe eating disorders. I’m thinking I’m going to have to eat separately. Hopefully I can inspire by example.

I have read something about ageing, sleep deprivation and chronic stress causing reduction in the body chemistry that signals satiety when eating for older people. Can you please comment on this, as this is a huge issue for working mothers, and older women are now additionally at one of the highest risk cohorts for poverty and homelessness despite their essential contributions.

I've been eating the leaf of the turnip green (75% of food intake) and the weight is falling off. This is totally unexpected (lost 20 lbs in 2 months with no increase in activity). Hope continuing this will lose me another 20 lbs. I've had HIGH uric acid for 20 years (who knows maybe longer).

My doctor gave me allopurinol and it made my gout so much worse I was literally crippled in both feet for months and at a level of pain I could no longer handle. I read about the drug "initially" causing more gout but mine continued to worsen so I stopped taking it. It took weeks after stopping for it to begin to lesson and the permanent damage is significant. I'm not sure what to do not to deal with this painful and frustrating condition.

Question for Dr. David Perlmutter or Dr. Casey Means: It is safe to take BOTH supplements (Luteolin 10mg & Quercetin 500mg) daily to prevent over production of URIC acid? Or should one be choosing one or the other?

Fantastic discussion. I would love to hear more on the application of these concepts to those of us who are already well along the path of destruction. Specifically in my case with continuous A-Fib and rather advanced cirrhosis due to NAFLD. It is obvious that the nutritional regimen needs to be fixed but there are detailed issues for example with extended fasting and cirrhosis.

Love the overall message. Obviously the title of this video is at least misleading, since no peer-reviewed literature exists stating a causal link between hyperuricemia and dementia (and dementia as a construct is vague anyway). Proposed mechanisms linking the two are compelling, though. Yeah, sugar in the forms of concentrated processed sodas and candies? Steer clear.

I was blessed enough to be treated, twice, by the Dalai Lama's personal physician, in India. He is one of the leading specialists in Tibetan medicine (see the Blue Buddha). It is very similar to Chinese medicine. He recommended eating "warm foods" after 4pm. Heavy nutritious food such as lentils. No fruit. It is a "cold food". These are just two examples. This is related to the inner core body temperature. Tibetan medicine has been practiced and peer reviewed for 2500 years. And it works. I wish western medicine would start following the simplicity and "you are what you eat and think" advice from the specialists in this ancient knowledge. But then pharmaceutical companies would lose billions of dollars of profit every year.

I was confused by the assertion that uric acid is bad for your health while enjoying the incredible decrease in arthritic pain, due to tart cherry caps. I looked into it and it appears that what's being referenced here is serum uric acid. This is purportedly reduced by the consumption of tart cherry juice.

Benjamin Franklin in his youth was a known athlete(trekking, running, swimming), he lived well beyond the average age for his time, yet in the latter years of his life he was crippled with gout. I tend to attribute this to his rich diet while living years in France. - I believe he would’ve welcomed and heeded Dr. Perlmutter’s discoveries.

Man wish I knew about this Diabetes way back when and learn more about it. I was really bad at drinking Coke a cola. I was drinking at least a 2 liter a day if not 4. Now at 53 I got High Blood, Cholesterol, Diabetes, and got really bad nerve damage in my feet and I get Gout too. Not sure if I would have listened but my father had died of Diabetes and he hardly ate sweet stuff like he have a piece of cake on someone's b-day. But he drank a lot of Coffee and sometimes soda but it really wasn't his thing. I and my Mother on the other hand ate all kinds of sweets and she was a baker for all kinds of parties. At Christmas time she would make loads of cookies and all kinds too. I don't think I have Dementia but I don't remember a lot of things just certain things. I got Glaucoma in my eyes and I can't read things without Glasses. I'm great at eating fresh foods and dinners that we make not out of a box or a can except veggies of course. But I do sneak in a treat here and there and some soda which I have cut down on it but I still sneak it in. It's hard to give up some things that you like to save yourself the pain but trust me nothing like numbness in your toes and certain parts of your feet and all of a sudden your up all night cause your feet are in electric shock mode and can't stop it from being still. Once you have it it's a different lifestyle you got to live in order for you to not feel that pain. Sometimes I feel like grabbing the sledgehammer and smashing my toes cause of it. Funny that I rather smash my feet than to give up the stuff that is hurting them in the 1st place. How the mind works.

I have been gout free for 1 year 6 months. I started taking MSM at least once a day. And 2 big spoonfuls of Psyllium seed powder, which soaks up uric Acid like a sponge. My Psyllium seed powder lowered my Uric Acid from 9.8 to 6.4 in just 3 months. Methylene Blue is also very good for healing metabolic disorder which gout is. Which is also why CoQ10 helps so much with gout. But Methylene Blue is alot more powerful and alot cheaper than CoQ10, though I take CoQ10 also. Any way, this combination has been the cure for me.

I've just spent 12 months of gout hell. I'd had maybe 4 gout attacks in my previous 47 years. Then last year happened 😳 I had a massive attack in my left upper foot in October that lasted for 3 weeks. Then in November I had crippling gout in my hand and wrist. Absolutely destroyed me. It took 3 months of hell before I could even lift my hand without pain. I work in China and good medical help has been so difficult to get. I finally got put on Febuxostat in February 2022, one pill a day. It didn't do anything really, I then continued to have a gout attack every 10-12 days for the whole damn year!! 6 weeks ago I made the decision myself to increase my dosage to two pills a day (I researched on the official website and that's what I should have been prescribed all along). I'm now on day 25 without an attack and am feeling like I've finally started to turn the corner. The doctors did give me an apology last week, they realised based on the fact I'm not a Chinese sized person they should have adjusted the prescription. Anyway no hard feelings, I'm just so relieved I can finally start to move forward from my shitty year!!

Later on in my 30's more gout, then in my 40's blood clots, and finally now that I'm in my 70's found out I've had type 2 D and now done with fructose. Losing weight and doing great. Wish I would have known all my life what the effing sugar and carbs did to the body. UGH!!

This is a great research area. This rightly is getting much more attention. A subtle behavior of uric acid is how uric acid rises during prolonged fasting (>24 hrs). The uric acid behavior during fructose consumption seems to be related to fat storing, but during fasting, fat is the primary fuel. So why does uric acid rise? Uric acid has to be doing more than storing fat. Perhaps it limits fat wasting. Keeps lipids from leaving the body.

This is all so interesting and I’m so grateful for this interview. However I’m at my wits end because I need a regimen -something that gives me a specific diet and recipes of foods I can cook and serve my husband that suffers with gout. Everything has purines. He is 63 and has chronic gout. He has tophi in his hand-several knuckles and middle joint of his right index finger and I suspect in one Achilles’ tendon which flares up every couple of months. We’ve tried keto and initially he lost about 20 lbs but gained it right back when he wasn’t as strict. His gout has steadily gotten worse. He has recently gotten on allapuranol (sp?). It has helped range of motion but he had a flare about a month ago. Is there anything that can be done about the tophi? His hand and finger are so painful. 😭😭😭😭😭😭

my Ex was 22yo, 6ft & 165lbs, Excellent diet & fitness (was very thin) but had very high levels of uric acid and BP of 280/160. Said he was near having a heart attack or stroke. it turned out he was born w 1 kidney 1/2 the size of the other with 2 arteries going to it. His body (as a fetus) knew the kidney was small so it grew another artery to it & kept his adrenalin going to pump more blood. Blood pressure meds made him so he couldnt get off the couch, "the winds have been taken out of my sails."

Brilliant podcast, thank you! I'm unable to afford a visit with a functional medicine doctor, so like most Americans, I have traditional insurance through my employer and see a PCP. I've questioned the past two PCP's I have seen....please test my insulin levels and cortisol levels....they tell me "insurance won't cover it and it's not necessary...you're fine." I'm 55 yo female and never had high blood pressure until 10 years ago....slightly heavier now, moderately high cholesterol, osteoarthritis in hands two years ago....I don't understand why traditional medicine isn't on the same path as functional doctors!!! You are all so spot on! I have been working with food to help reduce inflammation and help gut health. I have belly fat and I know my liver is mildly enlarged. Why won't doctors listen to us????

Everyone I know who eats a high carb diet is fat and suffers from some health condition despite leading relatively healthy and active lives. But I have one cousin who's on the Keto diet and has avoided sugars and carbs for several years, and he's the healthiest person I know. And this is a guy who drank and used drugs for two decades. I was diagnosed with type 2 diabetes end of July, I had non alcoholic fatty liver, hba1c 7.6, uric acid was high. I stopped eating rice, bread and pasta cause I was controlling my blood sugar. I lost 20lbs in three months. Last blood work first week of December, my uric acid is in the normal range, no more fatty liver and hba1c is 5.6. I didn’t take any medicine for my uric acid but I did take a uric acid cleanse supplement. It takes a lot of self control but my fear of diabetes complications keeps me in control.

Very very interesting! A unrelated topic when it comes to health is, the colors u wear. Black is a complete lack of color. Red is emotional, green is the growing color, light blue, light yellow & white are spiritual colors. For those who love to wear black, this is a heads up, that it’s NOT good for that protective sheath around you, which protects your space, your aura.

My dad was the healthiest person I knew. He became a vegetarian before it was trendy, over 30 years ago. He also watched/limited his salt and sugar intake.
Despite all his efforts, he passed away a couple years ago from a heart attack, and before that, struggled with high uric acid for decades. Gout was very debilitating for him. It was hard to see him suffer.
Every since he passed, I’ve been trying to understand what happened to him. How did someone like him get hit with gout weekly or how were his uric acid levels always elevated???

Uric acid is a cause for kidney stones . I had kidney stones and my urologist tested me and discovered my stones were uric acid based . He prescribed ( potassium citrate ) and that cleared up my problems. After many years I ran out and stopped taking the prescription and I developed kidney stones again ! Needless to say I'm back on potassium citrate. That was a very painful experience .

These days, my uric acid level is under control (5.5 last tested) and it's harder than ever for me to lose weight. I'm not suggesting lower uric acid or Allopurinol made it harder to lose weight. But I am saying it didn't make it any easier.
I've never heard of the connection of uric acid and weight gain before. And I'm not convinced of this now.

I had my first gout attacks this past winter. Didn't know what was happening at first and thought it was beginning arthritis. Researching my symptoms I then stayed away from the typical foods and drink that exacerbate the condition but still had another flare-up. I then researched my medications and discovered that my blood pressure medication, a diuretic, can cause uric acid levels to go up, plus delete potassium and magnesium, which no doubt contributed to my episodic severe depression. Neither the literature attached to my prescription nor did the doctor warn me about uric acid or mineral depletion nor were those things checked in my blood work. The latest gout incident happened when I used Splenda in my morning tea and yes it can be a trigger. So definitely have been on a learning curve and learning more thanks to your video!

A big issue is how to help people through the process of letting go of these foods. Many people fail because they mistakenly believe that they will lose the desire for them just because they know the foods are damaging. Of course, really believing or accepting that they are can really help. (But it took for me to see what was happening via an expensive out-of-pocket bout with a CGM to make it easier for me, though I had cut sugar to no more than 10 added grams- yeah, I know we don't need ANY but having some dark chocolate kept me on track at first. Now I make my own and rarely have any with sugar.) And yes, some people lose all craving pretty quickly but that is NOT universal and we do people a disservice to promise them that. I know of one woman of a site I've used for using meal-based eating who has been circling the airport, at about the same level of obesity for nearly 12 years because she is convinced she will find a program that takes away all her desire to overeat. At age 69, I actually don't have a lot of true hunger, but I am still very interested in eating, and I have been in maintenance for over 10 years. But health issues have kept me refining my choices.

it would be so helpful if someone or a company makes a product/pill/etc that would have all of these particular ingredients and sell it. there are so many vitamins, supplements ,etc and it does get very confusing for a lot of people. I personally would be happy to buy something that contains many or all of these ingredients so that I can purchase them for friends and people I know who are in desperate need of help with gout, etc.

This has all been very interesting and honestly enlightening, connecting previously disparate facets about key chemicals and processes in human physiology.
One thing I'm curious to learn more about based upon this are the implications of salt during medium term fasting (1-3 days). It's generally recommended to make sure to have a good amount of salt along with high water intake during fasting to ensure we have sufficient electrolytes to function. However, based on the info in this video, salt intake triggers fat storage, which goes directly against the goal and often core benefit of fasting, which is to burn existing fat. Of course fasting has other benefits, but I would really love to hear a deeper illumination of the signals and pathways involved in salt intake and fasting as it relates to fat burning, or lack thereof. Thanks!
And thank you for this fascinating discussion - I will pick up the book!

I appreciate the callout to LMNT and the need for conditionally needing electrolytes. This helped clarify the "salt makes you fat" talk up to that point. As a sauna and keto diet user who is very physically active, salt is a focal point of my performance & recovery.

I recently did my blood test and fasted for a longer period than the typical 12 hr (did 20hrs). My blood test results came back to show that my uric acid levels at 8mg/dl. I've only one gout attack in June 2020 and have been reading how to avoid it. I've BMI 22, blood pressure is normal, been exercising 3x a week for at least 6 months, did the occasional intermittent fasting and cut down on sugar. Given 2/3 of uric comes from within the body and my mum has history of gout, how should I be managing this elevated levels of uric acid despite the life style changes? Should I take vit C, quercetin supplements? Hope this esteem group of listeners, Drs and clinicans and lend some insights.

I used to be very overweight. A low fat, low calorie diet didn’t really suit me at all - I lost some weight, but it seemed to be as much muscle as fat and left me weak, tired and hungry. I tried ‘The Atkins diet’ which was fashionable at the time and it worked amazingly well. 15 years later my weight is still under control and I am very fit and healthy. I remember at the time Atkins getting almost universal condemnation from just about everyone, everywhere in the medical profession or media. The ‘fat bad, calories bad, carbs good, fruit good’ mantra was not to be challenged. Seems Atkins was definitely on the right path, even if he didn’t have the full story.

My husband had uric acid kidney stones a bunch of times. Was on alopurinal a few times but then went off it. In his last few years he tried paleo diet to lose weight but would stop because of back pain and missing carbs. So back to carbs. This back and forth wasn’t helping as he was eating high fat then switching to carbs. He thought he had rheumatoid arthritis like his mom but in fact it was probably gouty arthritis. His last docs didn’t check uric acid levels and l believe he died from complications due to uric acid and diet. He probably had a heart attack at 70. It was so sad to lose such a good husband and father. This lecture reinforces my bit of research l did regarding his death.

I always knew that my dad's last meal was responsible for his death and now I understand why. He did have gout. On the night he passed he had eaten a large portion of sauerkraut with pork. My mother told me when he awakened not feeling well, he drank a glass of orange juice thinking it would make him feel better, but he immediately died right there. I now understand why I instinctively knew the combo killed him and now I understand why. This is very valuable information. Thank you so much.

Please correct me if I'm wrong, but if we consume sugar containing products with a meal ( protein, fats, low GI carbs eg veg) the absorption of sugar is slowed down significantly enough to reduce peaks of glucose (or fructose) in our blood and so reduces Insulin release and therefore must reduce Insulin Resistance.

Fascinating. I would surmise that the 5 apples would be fine as long as you aren't eating the refined carbs and drinking lots of sugar ridden drinks. I've been looking into raw fed vegan and fruitarian diets with seeds, nuts and avocados as fats. I don't have weight issues and exercise quite a lot but I know people who hardly eat and still gain weight but they drink a lot of cans of coke ect so that makes sense. I've often said to one friend I think its because her body stays in a stressed state of starvation mode. It's really interesting to hear the relationship to Uric acid in all this.

Drop Acid Highlights From Dr. Perlmutter's Book On The Dhru Purohit Podcast

I wanted to highlight some important points that Dr. Perlmuter makes in this podcast with Dhru Purohit. This is a fantastic podcast where Dr. Perlmutter explains some important distinctions about the effects of fructose and uric acid on metabolism which he writes about in his book Drop Acid. The fructose-uric acid pathway to fat storage is distinctly different from the glucose-insulin fat storage pathway. You need to know about both in order to control metabolic syndrome.
ABOUT THE HOST: Mary Beauchamp is a Registered Nurse, Best Selling Author, Ketogenic Diet Specialist, and Certified GAPS Practitioner (Gut And Psychology Syndrome). Interesting.
Does uric acid actually cause fat storage?
Or are uric acid and fat storage both the result of fructose?
fructose—> insulin —> fat storage
Is there an interventional study that somehow verified that uric acid is the cause?
Or just correlations?
Does he explain the mechanism?
Does that explain why apple cider vinegar helps with weight loss, since it increases uric acid excretion by kidneys?

I interview New York Times bestselling author of Grain Brain, Brain Maker, and Brain Wash about his new book, Drop Acid: The Surprising New Science of Uric Acid—The Key to Losing Weight, Controlling Blood Sugar, and Achieving Extraordinary Health.
Dr. Perlmutter shares his insights into why this obscure blood measure, uric acid, formerly thought to only be relevant to gout and kidney stones, is actually a valuable and precise regulator of metabolism and fat storage. He explains how gaining control over your level of uric acid adds great advantage for weight loss and other health benefits.

I watched a YouTube video a couple of years ago about a man who suffered from gout. He tried the usual remedies by first cutting down his intake of meat and then by stopping drinking alcohol. It was not until he removed fructose from his diet that the gout went away.
I have a friend who manages her gout by drinking cherry juice.

I might eat fructose once a year .. a little ice cream, breaking the keto / lchf diet for 1 day out of the year,.

I have a question about daily aspirin use as a preventative measure for heart attack or stroke. My elderly mother has been on aspirin for several years and I’ve never really understood how this can be a good thing. She always seems to have bruises, has not previously had heart attacks or strokes. The reason for the aspirin was, if she were to have an episode, she’d be asked if she was taking aspirin and, if the answer was “no”, then she’d be prescribed it as a first measure. In order to be ready for the step, the recommendation is to take aspirin so that if anything happens, she can move to the next step. This has never sounded like a good plan to me but, I’m not a doctor. Mum had a minor blood clot in her mid calf up until 2014, it disappeared after a hip replacement surgery and treated with specific medication. The blood clot has not recurred. It was the sort that wouldn’t travel but, it’s gone but, she’s been on daily aspirin in case she has any sort of episode that requires aspirin as the first step. What are your thoughts on aspirin as a preventative measure for the rest of one’s life?

And like a typical doctor misses the fact that you need nutritional minerals to regulate blood sugar not a herb herbs were so important they would have to be all over the crust of the Earth every human being requires all the same nutritional minerals at about the same time there's five major minerals that help regulate blood sugar and probably four primary vitamins... But you need all 90 nutrients everyday

So he makes this claim about uric acid that is unsubstantiated. Then he backs it up with all the things that people have been saying for years. Your health is your responsibility and how healthy you are is the result of a constellation of different factors.
